Incredible amounts of data being harvested as China tries to track travelers and the coronavirus. On the high speed rail everyone has to download a miniapp and fill in their info including address, name, id, phone. When you get off workers check phones to make sure you submitted.
This was in Shanghai. If you choose to fill out a paper form staff check your ID. They said they have a team of people to enter the data from the paper forms into a digital database. Here @lqq91926 tried it out. 
One point here is that the system to track people isn’t working all that well if it requires this sort of extra help. The second point is what long term happens to all this data? Based on precedent I’d bet it won’t get deleted and also won’t be securely stored.
