1. Well, I'm not a CNN shill, but I was an FBI agent, and I did work on the JTTF. And during my tenure applications for FISA warrants were meticulously written, verified, and reviewed at at least five levels before getting to a judge.
2. Usually the agents writing the application had it returns to him or her many times for clarifications, for additional evidence, etc. That is why there was a high percentage of acceptance when it got to the court. It's obvious the Page affidavit did not go through that scrutiny
3. And the judge was either hoodwinked or compliant with the scheme.
