Had a great time at last night's #BAAHM meeting, discussing how to use Twitter to stay informed, as a #meded tool, and for professional development.

It was awesome facilitating alongside @AdamRodmanMD and @moss_caitlyn
Here are some "Twitter start-up instructions" we shared with the attendees. We encouraged everyone to complete this beforehand.

Next week I'll be giving a talk at the Society of Hospital Medicine Annual Conference (#hospmed19).

A 60-year-old man with hypertension and hyperlipidemia presents with substernal chest pressure. After initial evaluation in the emergency department, including a normal ECG and one negative troponin, he is admitted to medicine for “ROMI”.

I've cared for thousands of patients on aspirin; few have petechiae. Why?

Let's examine the tiniest of the blood cells, the platelet. Turns out, they do more than just stick to each other...

 A question: which of the following explains why aspirin rarely causes petechiae?

TA₂ = thromboxane A₂
VI = vascular integrity
AJ = adherens junctions

 To understand the different manifestations of quantitative and qualitative platelet disorders, let's first review how thrombocytopenia might lead to petechiae.

Though the connection between platelets and bleeding has been observed since at least 1881, we'll start in 1966.

 Why do thiazide diuretics cause more hypokalemia than loop diuretics?

Loops DIRECTLY affect potassium (via inhibition of the Na-K-2Cl co-transporter), while thiazides affect the Na-Cl co-transporter (no K!)

And yet, thiazides cause more hypokalemia.

Let's find out why.

One study reported the following decrease in serum potassium for each diuretic type:
*thiazides = 0.66-0.68 mEq/L
*furosemide = 0.19-0.36 mEq/L

Last night I dreamed I was eating soup at Twitter HQ with Jack Dorsey. I don’t think this reflects well on my mental health.
This might seem like a stupid dream but actually I spent a day at Twitter (even thought about working there) several years ago so I guess dreaming about it isn’t so weird. Not sure what the soup means.
Anyway in this (very weird, serious dream, wtf is wrong with me) I was asking Jack why the hell every other platform with Twitter’s reach has branched out into messaging, payments etc. but Twitter has basically added 140 characters.

Okay, so I need to talk to you about something. Something I committed to being more open about a few years back. Living with anxiety. This is the ‘launching a book whilst having an anxiety disorder’ thread.
So the last couple of weeks I have barely been on Twitter, just popping in occasionally to RT a nice mention or two, or reply to people. This isn’t the way I like to use Twitter. I wasn’t being very social.
As today approached, I was aware of an increasing pressure building: I should be online more. I should be more social. I have a book coming out soon! No-one will know about it! Work harder! Promote! And of course the subtext here was: You are failing.
