Outlining priority preparedness measures for possible pandemic spread of #COVID19. World is unready for that scenario, and we have a shrinking window in which to prepare.

Key priorities:
▶️Scale up support to frontline health workers
▶️Mitigation strategies if health systems become overwhelmed
▶️Reinforce supply chain of PPE and other critical med supplies
▶️Risk communication through credible experts
▶️Invest in sustained US preparedness
▶️Support and finance readiness in the developing world
▶️Empower an int'l coordination process
▶️Support research and tech innovation, particularly for diagnostics
▶️Begin planning now for production and distro of eventual medical countermeasures
We don't know yet if conditions in China will be replicated elsewhere. But it's prudent to prep for that contingency, even while hoping against it. These measure provide a starting point.

- Not at all under control
- Community transmission is likely
- May become endemic like seasonal flu

@CDCDirector In perspective: best available data suggest this virus is ~2x as transmissible as flu and ~10-20x as deadly. Community transmission could be extremely dangerous.

@CGDev and expert colleagues propose priority measures here:
@CDCDirector @CGDev Over the past few days there has been a distinct shift in USG messaging on COVID19 - no longer "risk to the US is low." Much more frankness on what could be coming - very constructive. Kudos.

Had a very concerning chat with a hospital exec recently about preparedness for nCoV.

US hospitals nowhere near ready for conditions like we're seeing in China. Neither are those in the developing world.

If anything, this NYT report may be too rosy.

Can't say for sure yet that China-level spread will begin appearing elsewhere. But plenty of reasons to think it could. We shouldn't take too much comfort in the low numbers we're still seeing outside of China. 👇
US hospitals are running full-tilt with flu season already, and beginning to face supply shortfalls due to nCoV-driven surge in PPE demand. Major supplier of N95 facemasks has told hospitals that ability to fulfill orders will be questionable for next 4 months.

Ruth makes this point, rightly, every year.

By this stage, @GiveWell's recs start to look more like a persistent bias or limitation inherent in their model, rather than a credibly objective take on different charities' effectiveness.

Their focus on where a next marginal donor dollar can do the most good is appealing, in theory.

But it presumes that's a useful way to measure effectiveness.

In practice this = heavy bias for simple, linear interventions that perform similarly across diverse environments.
That describes a subset of dev projects that are amenable to linear, quantitative measurement - mostly health-related. The model consistently rewards them.

But plenty of dev projects - indeed most! - aren't linear. Particularly for @GiveWell's other target - poverty alleviation.
