April 5th outlook:
The US fatality curve crossed Spain's to be the steepest in the world, but even its slope is less than recent days. It's the beginning of a flattening, hopefully
New York is still the outlier; other regions a bit better (LA, FL) or worse (NJ, MI, CA, WA)

The case for US hitting its peak slope is furthered by other data from earlier today. It'll take a few more days to confirm.

The April 4th data for the US death curves

There is some modest decreased slope; we'll take what we can get but it is a sign of encouragement
New York's curve for April 4th remains a distinct outlier
NJ, FL and WA steeper
LA and MI somewhat improved
There are 2 ways to look at the US fatality curve today
Pessimistic: It has now reached that of Spain's and represents little change
Optimistic: It could have maintained the same slope (purple line) but it didn't.
We'll know which of these plays out in the days ahead

I was there as a hospitalist doing a month of nights. What follows is my experience and what I learned.
You know how people say “my dad was in the war but he never talked about it?” These memories are like those. They have been sort of locked away for 10 years.
We didn’t have the forewarning like we do now. We just heard there was an early flu season and the flu was worse. I feel like the beginning of the month was normal. Chest pain. Pneumonia. Pyelonephritis. Asthma exacerbation.

The basic idea of the Rule of Halves is simple. Of the people who have a chronic disease:

~50% are diagnosed
~50% of diagnosed are treated
~50% of treated actually see a benefit
What this means is that only a small fraction of people who have a chronic disease actually get treated to the extent that we would like

And it's a huge problem for some diseases
