In brief, data from several meta-analyses show that #HCQ with or without Azithromycin is not effective in reducing mortality to #COVID19 or its complications.

Our updated meta-analysis of RCTs using strict inclusion criteria of 1) controlled studies ....

Solutions to lower the case number, avoid a second wave and at some point reach #CoronaZero:
1) Generally, we need to be #proactive and #creative in our thinking and use our massive scientific and financial resources fully in a targeted and smart manner.
There is no typical Swiss compromise with this virus, this is a fight for our health and economic prosperity like we have not seen since World War II. We need to be humble and learn from the countries that are doing better than us.

Fascism is a word so often misused, that it immediately elicits eye rolls from serious ppl.

But it has a meaning: ultranationalist, dictatorial gov’t that deploys fear of the other, both inside & outside the nation, to consolidate power.

Trump is accelerating fascism.

Think about the way Trump talks about #BLM protests as an “anti-American” Force, not out to promote racial equality, but to tear down American “heritage.”

His deployment of CBP paramilitary on the streets in #Portland is a profound escalation acting on that narrative.

Consider the fact that he continues to call #COVID19 the “China virus” and stoked conspiracies that it had been developed in a lab in Wuhan to reinforce the narrative that they are the enemy abroad.

There's a group on #Flickr whose title always gives me a laugh: "Narcissism in Color". When I make selfies, I have to admit that I am very much like Narcissus of mythology: the person who looks at them the most is me. This audience leans over the edge of the pool and dreams.
There's a strong association between #narcissism and dreams in #mythology. Many of us dream of that face in the pool, giving it fame that we think we deserve. The Instagrams of young people attract many likes. But this is no country for old men.

No #mask was used. A student spectator observed droplets of saliva comin from surgeon’s mouth while he spoke to class & conducted his operation-shared it with Dr #AliceHamilton 1/4
Dr Alice Hamilton, teacher there conducted research on it-She kept a culture plate on table while surgeon operated & spoke upto 500 words an average of 75 bacterial colonies were identified which contd Streptococci, Diplococci & other bacteria- half of them were harmful (2/4)
She also observed:
-Speaking released droplets up to 24 cms
-With coughing it was 36 cms
-Whispering produced more droplets than talking

She concluded that this spray of saliva thru surgeon’s mouth can cause harmful wound infection to the patient getting operated (3/4)

1) National leadership, starting with POTUS, for dismantling pandemic preparedness, underfunding public health, labeling #COVID19 a hoax, bungling testing, refusing to model good behavior (e.g. masking), pressuring states to reopen too early, etc.
2) In NC, the General Assembly GOP majorities #ncga #ncpol, which have refused to provide adequate funding for public schools (and basically every other public good), repeatedly downplayed the severity of COVID, refused to model behavior like masking, pressed for early reopening

1/ We should consider the #mask like a #condom as it blocks viral transmission. Promoting condom use vs AIDS also helped ppl understand the other safe behaviors to be adopted e.g. sticking to 1 partner. Likewise effect depended on what other person was doing. Here's tips how👇
2/Also similar to the AIDS epidemic, there is asyptomatic spead & you cant tell who's infected. Likewise it was common that people made assumptions about who's safe to be with based on how they look & if they know them ("it can't be him, hes a great guy", "she looks ok").
3/ Lethal mistakes bc of assumptions & not u/standing transmission. Despite this, in the studies we at @WHO did on condom use in dozens of countries, #1 predictor of use was not of knowing someone who died, believing in lethality, or any info, but what they thought peers did.

Why was COVID19 testing capacity limited early on? Not only did machines need to be purchased, processes validated, supply chains were strained – nasal pharyngeal swabs, pipette tips, wipes. Even now, reagents are in short supply. #EORLA keeps problem-solving.
In 2 months, #EORLA completed as many virology tests as normally completed in 2 years. Continuously increasing the number of tests has required totally new processes, work flows and ongoing adjustments. The virology team has grown from 6 people to 29. @CHEO @OttawaHospital
Every single COVID19 testing kit is put together by a small #EORLA team in one windowless room. 100,000 (!!) as of June 11, with Lynda supported by Erin overseeing the work.

What does science find about use of #masks ?

Here is a meta-analysis:

However, N95 masks are worn in a surgical setting. Why?

Because bacteria are at least 10 times bigger than viruses, and can often actually get trapped in the tight weave of a N95 mask. This helps prevent hospital-acquired infections such as staph & strep. But, . . . 2/
But viruses are much smaller and easily pass through N95 masks, let alone masks with much larger pores, such as paper surgical masks and cotton masks.

In fact, the pores of cotton masks even enlarge after washing, and are then even less of an obstacle than a #coronavirus, 3/

1/ @SickKidsNews released guidance today about school reopening. Colleagues, mentors +caregiver advocates among co-authors. I was not involved in its development, and can appreciate concerns raised re: content and comprehensiveness. A few thoughts as peds + ID/IPAC physician:
@SickKidsNews 2/ This was a summary of current published evidence + #IPAC principles to consider w/school reopening, and would've benefited from ON-specific data - BUT we couldn't routinely test sx'ic kids for MONTHS unless they were admitted. Huge gap in knowledge of provincial epi.
@SickKidsNews 3/ Clinicians are trusted sources of knowledge + community partnership; we have a role in knowledge translation. Need scientists at table to dissect real-time data + model impact of strategies (screening/cohorting/shielding) to inform dynamic approach ie responsive to local epi.
