Back to reporting after ~4 weeks of battling the virus, being a caregiver to 3 in family, finding hospital beds, labs, struggling to seek info about hospitalised parents. All this in a small town. Was lucky though to have eventually found resources & recover. Many are not #Thread
The chaos, health infra in JSR, my hometown & a fairly developed city, has given a 1st hand experience of healthcare in small towns. Doesn't end at lack of infra. Sheer negligence & under-preparedness both by govt & ppl is beyond belief. Story same across tier 2&3 towns in India
#Covidcrisis has fiercely moved from big cities to small towns - those having limited experience of 1st wave, were complacent & poorly educated about the outbreak. Health infra broken, unable to take the load, processes are slowing. 2020 was just taken as a one-off historic event

While India is witnessing a surge in COVID-19 cases, the Defence Research Development Organisation (DRDO) has set up a facility in New Delhi with all basic requirements, free of charge.
The COVID facility with all oxygen beds, a large number of ventilators and air-conditioning is set up by DRDO complying with the World Health Organisation (WHO) standards.
If a patient suffers a neuro or cardiac case then the patient will be referred to the All India Institute Of Medical Sciences (AIIMS).

Hospitals 🏥 & Doctors 🥼 are unable to detect cause of illness. Instead they are treating every patient as covid patient and giving them many doses of injections and medicines resulting in death of patients (1/n)

2. Why are remedesvir so important ? Why doctors are prescribing only that dose ? Isn’t there some other alternative or other dose which is somewhat similar ?

I have been curious about the changes to ICU capacity capacity. The weekly data captured at healthdata.gov/sites/default/……… now shows data through the week of 12/11. At first blush, the data doesn't appear to have changed much since the prior week. 1/n Image

First up, hospital Onset Covid. These are the # of people currently hospitalized who were in for 14 days for something else, but got Covid there.

Inpatient Utilization. This is the percent of beds that are currently filled. Currently running at about 75% capacity. This hasnt changed much across the last 6 months of the pandemic, regardless of how many Covid patients are hospitalized

ICU bed utilization (These are staffed beds), along with Covid utilization. % of ICU beds used for Covid has increased, but overall utilization has only risen by half. Does that confirm that some of these are people who would be in the ICU regardless?

The first being the choice of managing #COVID19 for hospital capacity instead of aiming for #COVIDzero.
Then within the management regime several others failures. First: insufficient testing. Frequent testing at #LongTermCare, and #hospitals would have saved many lives. A large scale deployment of neighbourhood testing centres for anyone who wants it woudl have curbed transmission.

One of the main reasons we moved back was the deteriorating health care system in the US.

Everyone needs to understand there is a HUGE difference between "health care system" and "Medical Treatment".

"Medical Treatment" is what doctors do. Be doctors.

Diagnose. Treat. Cure
"Health care system" involves the huge mess surrounding the doctors under which they have to operate.

The layers and layers of bureacracy, third party payments, MBA bosses, pharma companies, medical malpractice, corporate work culture, VC partners etc.
Needless to say, the layers keep on increasing and move the patient further and further away from the doctor.

So now, the diagnosis happens only if the insurance company approves the check up or the tests or imaging.

The diagnosis has to happen within a stipulated time

Woah..that was freaky but familiar. Was lying down & decided to get up to take pain meds for #endometriosis and #fibroids and it felt as painful to get up as the day after surgery. In kitchen, sweating, lightheaded. Quickly squatted to avoid passing out holding onto fridge door
..which swung open..but I did not fall. Then once head feeling better, stood up long enough to take meds...then walked, wobbly and lightheaded to chair. I can't quite feel my palms but that will come back. This probably gets to be called 10/10 pain..but maybe really an 8/10
Great..and a migraine too..scotoma.. taking a migraine med and lying down. Man, when it rains it pours.

Mother Nature is such a misogynist.

SO glad I did not decide to rent a car and try to go Finger Lakes this weekend to see peak foliage.

According to a media report, German hospitals are keeping fewer and fewer beds free in their intensive care units for patients who are seriously ill with the novel corona virus, despite the increasing number of infections.
