The guy wanted to leave AMA so his nurse and I spent 20 minutes at the bedside identifying every single thing he didn’t like about being in the hospital and figuring out if we could fix or change it.

Then I gave my usual line: “I’m not the warden and this isn’t a jail.” 1/x
He said, “I appreciate your efforts but I want to leave.”

“No trouble at all. Let me get your things in order and make a plan that might get you through the next few days.”

An hour later he decided to stay.
Honest to god I don’t actually care if any patient stays or goes. I’ve seen full grown adults make worse decisions and took responsibility for none of them.

I invested this time to learn about my hospital. What are its weaknesses? What’s frustrating and uncomfortable?

- pain
- disappointment
- uncertainty
- loss
- fear
When you experience these “unpleasant” things, consider the patients you care for. They have been experiencing them too. You might never have known to ask, though, until you have experienced them yourself.
Your own suffering is a gift. It’s a window into the experiences of the people you take care of.

For some reason today of all days as I was writing my notes I was suddenly irked...

I was writing “Acute Hypoxemic Respiratory Failure” and sufficient qualifying supportive verbiage and I considered the physicians who first tolerated being told what words to use in a patient’s note.
Surely they thought it was no big deal. Just standardize your words to help the people who pay you. Sounds so innocent. A minor request.

If you aren’t happy doctoring lately, it might be a mission problem.
In the US for-profit healthcare system, profit is certainly not the only mission.

It sometimes takes priority and taints the others often.
There are some places where you can practice where the profit mission is a less dominant priority.

Gonna go ahead and guess that a number of you did not accomplish all that you hoped to accomplish in 2018, and I'm here to tell you, that's okay. 2018 was the year that took ten years, a time-warp mind-fuck of a year that felt less like a calendar and more like a circus mirror.
What I can tell you, from some experience, is that in this regard the only constructive path forward is forgiveness of yourself. That's it.
You can try to blame yourself, or blame the year, or blame the news, but blame isn't a useful emotion, really -- it has you looking back with rancor and disdain, and though it can feel good to find a root cause, this isn't science. You can't really find the reason. Is what it is.

1. Medicine Hat is one of the most politically conservative places in Canada. Two years ago, the federal Conservatives won 79% of the vote; the NDP got just 1%, running behind the Christian Heritage Party. 1% -- that's not a typo: elections.ca/res/rep/off/ov…
2. Their local newspaper, the Medicine Hat News, is owned by Conrad Black's former partner, David Radler. You know his strategy: have three employees in every small town paper -- one to write filler stories, two to sell ads. And hire as cheaply as possible.
3. It's easy to hire cheaply, if you don't care about quality. There are literally thousands of laid-off journalists in Canada. But they probably have expectations of a living wage. So Radler -- and I'm not criticizing him, it works -- aims lower. Like this guy: @MHNJeremyAppel

There’s a lot of bigot vitriol surrounding how we “identify”.

When I say we, I mean members of the LGBTQ community.

When I say bigot, they’re usually white cishet middle class bigots who’ve got nothing better to do with their time than hate on people who are different from them.

2/
They usually have the same talking points that they focus on.
Things like “You’re all perverts and pedophiles” or “You’re all sex offenders & rapists” or “You’re all unsafe around NORMAL people & kids” or “You’re mentally ill” or “You’re sinners” or “You’re grooming kids.”

The good thing that Delia has done is being brave in coming forward on such a divisive issue. She has joined so many that are coming forward in saying she is a #remainernow - here ⬇️ is reason 2 for her original vote.
As Delia says here ⬇️ as someone who works in & cares about the #nhs she wanted to give it £££ (completely understandable) but like others she wasn’t aware this promise would not be honoured nor was she aware of the complexities like the Irish border!
