⚠️#COVID19 death toll in #NYC continues to outpace any other global region, albeit #China did NOT report accurately.

Some glimmer of good news, as the rate of rise appears to be FLATTENING in #NYC.

#Michigan not looking good, and #Florida is BARELY testing.

Overall US death toll is doubling ~every 3 days, BUT I caution that most states are NOT adequately testing *still*.

Fever data strongly suggest that our lockdowns ARE working, but cases/hospitalizations/deaths lag. Incubation period + time to get sick enough for hospital or ICU.
Another glimmer of hope: even as #coronavirus testing ramps UP, the #COVID19 case trajectory is starting to flatten.

Although we still have at least 4-6 weeks of hell to endure, the data show that our sacrifices ARE WORKING.

Last night in the ICU of a #NYC hospital, I cared for 20 patients who were all on breathing machines due to #COVID19. Some REALLY young (20s), without comorbidities. Everyone is extremely sick. But sicker patients keep flooding in…
We are REALLY feeling the strain. We don’t have enough ICU beds. Many patients are being intubated as soon as they show up to the Emergency Room. We are doing our best to take care of them on regular floor beds and in the ER itself. But it is HARD...
Some of our sickest patients with #COVID19 really need dialysis, but we don’t have enough machines. We are actively rationing care - something I thought I would never see in our healthcare system...

I told a 28 year old that he needs intubation. He was scared. Couldn’t breathe.

I told the wife of a 47 year old that he is dying over FaceTime.

I bronched a #COVID19 patient who mucous plugged. It saved his life. Risked mine.
I walked through the ED as the ambulances kept rolling in, bringing more patients with sats in the 60s.

I worked with an amazing team of residents, fellows, nurses and respiratory therapists.

I’m completely humbled by their heroic effort and teamwork.
I returned home on empty streets past shuttered store fronts.

In time to have dinner with my family for the first time this week.

Hoping that I won’t make them sick.

This is our new normal.

(Thread) While this chart is frightening, reality is even more dire. The contagion rate in New York City is actually around 3 per thousand (vs New York State's 2 per thousand). This places NYC just behind Italy's hard-hit Lombardy Region in per capita cases.@narativlive #Covid19
2. The fatality rate in New York City is also rising. The current rate is about 1.7%, a far cry from Lombardy's 10%, but as hospitals strain from overcrowding and frontline workers battle infections, the number of deaths will increase. That's why #Covid19 is such a killer.

⚠️#COVID19 cases in the US continue to explode, as I predicted over a month ago. The number of new #coronavirus cases is DOUBLING *nearly every 2 days*.

This likely is due to more testing and exponentially fast viral spread.

Expect this trend to continue for several more weeks.

(1/) Five days ago I forecast that NYC’s hospitals would be at capacity with #coronavirus cases by Mar 30. I was wrong. New data suggest that #NYC’s hospitals will reach capacity of approx. 3,000 cases by Mar 24. #shutdownNYC…
(2/) While there is now nothing we can do to prevent reaching capacity short of immediately adding beds, we can prevent additional deaths and hospitalizations by fully shutting down NYC except for absolutely essential services, like Italy’s current restrictions.
(3/) Why was the first forecast wrong by a few days? Detection rate. In the prior worst case scenario, we assumed an overall detection rate of 18%, which implied fewer undetected cases. New hospitalization data imply an estimated detection rate of just 5.6% today.

What is highly worrisome here is the #NYC seems to be moving into the ‘testing no longer matters as much’ camp. When the best science and studies are pointing toward the need for intensive testing. Oh well.
3/ I think one issue is that it is true once you’re to a certain ubiquity it becomes v hard to contract trace. But you need an intensity of testing to know directionally where you are and to find people who have infections but are asymptomatic.
