NEW INITIATIVE

Local action & ownership is the frontline of pandemic response. But there is little guidance out there designed for local leaders.

Over the past 2 weeks, a group of us have been working to start filling that gap: covidlocal.org
This project provides a starting framework to help local leaders orient and organize the COVID-19 response in their own communities.

Why is local leadership so important?

Because an outbreak is not monolithic. It varies greatly depending on local decision-making (e.g. 1918👇).
The tool outlines 7 initial key actions:

▶️Understand real-time local spread
▶️Slow and reduce transmission
▶️Protect high-risk groups
▶️Reinforce/expand health system
▶️Enhance risk communication

OK let's put this one to bed.

All the "China virus" and "Wuhan virus" stuff from Trump and Pompeo is a transparent attempt to divert attention from the administration's own failure to prepare.

It's BS.
Did China botch its early handling of this? Yes. Political pressure within China likely delayed the public disclosure of the virus by a few weeks. But still, China advised @CDCgov of the new virus on Jan. 3.

Did this trigger urgent action by the White House? It did not.
China was slow to impose control measures for the first half of January; however there was also considerable confusion at the time around the particulars of the virus. US officials at the time sounded positive about China's info sharing, with some caveats.

Lack of preparedness is a choice.

All of this - all of it - was an obvious possibility as soon as we saw what happened to hospitals in Wuhan. It is a damning indictment of federal leadership that this sort of guidance is coming reactively now, not proactively a month+ ago.
It was not hard to do the math on this. A paper I co-wrote over a month ago called for these measures. Our top 3 recs:
- Prepare guidance for overwhelmed health systems
- Scale up support to frontline health workers
- Reinforce the PPE supply chain
Addendum:

Have gotten a number of @'s asking why this lack of prep isn't the hospitals' fault. It's a fair question.

The short answer is - the economics of our health system strongly disincentivize this kind of preparedness without direct government action.

Back in January, no one expected a virus in China would explode into #coronapocalypse. My #travelinsurance company helpfully informed me that "fear" of a disease was not a covered reason for cancellation. The #cruiseship line wasn't offering refunds then either. 2/

Finally authorities are putting Mazandaran province on quarantine: only fuel and food trucks are allowed. This late, its effectiveness will be limited.

The same authorities have had argued for quarantine to be "non-scientific" and "from middle ages".
