Why does this happen? How can you avoid it?
I’ve thought about this question for a long time (probably out of fear it will happen to me). Here is what I’ve discovered.

We all know the product leader who releases a blockbuster & thinks they “get it” more than others. Ego.
They launch a chatbot product. Fails. They launch a crypto product. Fails. Their confidence starts to decline. Teammates lose trust.
They lose.
The Beatles started with simple rock-n-roll music.
Sgt Pepper's (8th album) created sounds that had never been heard.
They made use of orchestras & combined rock, music hall, psychedelia, traditional Indian & Western classical.
