I was sent these samples from a #hospital in #Chennai after the #physician caring for a patient wit abnormal #liver test found he was on these for #COVID19

Lead in drugs unheard of in developed countries. #India is developing (currently underdeveloped), hence an excuse. Lead limit in paints 90mg/kg.

Do U know how long it takes 4 a good liver 2 develop chronic liver disease/cirrhosis? 5-10 yrs. At times more. But here is a patient developing cirrhosis in 6wks. Yes. 6wks. Reason?Excessive #energydrink.Which drink?I cant say, but its a scary 1

 Here is the liver biopsy done at the time of liver injury and on follow up at approx. 6 wks. The top panel shows severe fatty liver and liver cell loss, damage/necrosis and the bottom panel, transformation to cirrhosis. Why did this happen?
Ans is a vitamin - Niacin or Vit B3

 ➡️Niacin, vitamin B3, is a water soluble, essential B vitamin
➡️Niacin is also found in many herbal mixtures and energy drinks, sometimes in super doses
➡️Well known cause of clinically apparent liver injury when given in high doses

 32 yr male. Routine check in 2017, found 2 have hepatitis C virus (HCV) +ve. That is, anti-HCV +ve meaning exposure to the virus & antibodies in blood.

 To confirm, PCR test 4 RNA has to be done. Which he did. The viral load was 900,000 IU/L. So he goes to a Gastroenterologist, who checks the virus genotype (it was type 2) and puts him on the fantastic direct acting antiviral Sofosbuvir+velpatasvir for 3 months. All good?
No.

 He checks Google. Finds cirrhosis, liver cancer etc...gets alarmed and before completing the treatment, checks HCV at 2 months. Finds - anti HCV still +ve.
[note: anti HCV ELISA test STILL +ve]
Feels modern medicine doctor cheated him with expensive drugs.

There R indexed and impactful journals charging #openaccess fee (eg @JHEP_Reports) if the institute can afford it. Most quality indexed, impactful journals (eg @HEP_Journal) dont have open access fee to publish, but #Subscription fee for persons to read #published study.
