It's 1984 and my mother arrives in the #UK with 89 other Vietnamese refugees known as the "boat people". With just the cloths on her back and her four children, she's confronted with the local people of a council estate.
Unable to speak English, she expects hostility and racism.
And then this happens.
A young scruffy looking man steps up, takes off his coat and handed it to the freezing cold refugees. A gesture so touching,
that everybody later followed. People then went home to fetch clothes they didn't need and handed it to the refugees and ensured they were all fed and watered.
My mother has never forgot that moment, when she was able to use a coat to wrap her boys so they could stop shivering.

3/ She could tell he was spent and getting sick himself. “Mark, let’s get you to the doctor!” “Not until you are well, Sharon.” He remained laser focused on her every need. As she became weaker, he took her to get admitted yesterday.

3 years ago, I was being put to sleep before surgery & while I knew everyone in the OR, I was frightened. I was fighting back tears on the table.

Today, a patient gushed to my attending that a fellow held her hand before falling asleep

The patient said that she normally doesn't get scared. She works in healthcare. She's seen it all. But something about being alone, naked, while people mill about you without seeing YOU was scary.

I know, I felt it too. I felt childish asking someone to hold my hand.
I asked her where she'd be going during her "anesthesia holiday", to which she replied a beach🏖️. No friends. No family. A few books📚. Teeny, tiny bikini👙. Mai tai's.

My attending smiled & said, "you realize the fellow who held your hand is Dr Muldoon"

Tips on selecting the right #OxygenConcentrator 🧵

The most important is to understand that #COVID19 patients require 90% Oxygen concentration at 1 to
5litre flow and above to 10L when they are suffering with acute respiratory discomfort.
90% oxygen concentration is the most important point here.
1. We can break down oxygen concentrator into small (5 to 10 kg) oxygen concentrator suitable for COPD patients, medium (15 to 19kg) and large (20kg and above) oxygen concentrators are suitable for critical care and for COVID patients.

Back to reporting after ~4 weeks of battling the virus, being a caregiver to 3 in family, finding hospital beds, labs, struggling to seek info about hospitalised parents. All this in a small town. Was lucky though to have eventually found resources & recover. Many are not #Thread
The chaos, health infra in JSR, my hometown & a fairly developed city, has given a 1st hand experience of healthcare in small towns. Doesn't end at lack of infra. Sheer negligence & under-preparedness both by govt & ppl is beyond belief. Story same across tier 2&3 towns in India
#Covidcrisis has fiercely moved from big cities to small towns - those having limited experience of 1st wave, were complacent & poorly educated about the outbreak. Health infra broken, unable to take the load, processes are slowing. 2020 was just taken as a one-off historic event

When the #ACA was implemented: there was no mandate established to ensure that #emr #ehr electronic medical records could communicate and physicians could obtain medical records from #doctors caring for the same patients at other offices or hospitals. 1/
What ensued was that the government forces practices to an incur most of the cost of buying expensive electronic medical records and forced them to use them or incur more financial penalties. In retrospect those who did not use #emr and #ehr benefitted 2/

Hospitals 🏥 & Doctors 🥼 are unable to detect cause of illness. Instead they are treating every patient as covid patient and giving them many doses of injections and medicines resulting in death of patients (1/n)
Instead of 2-3 dose , they re giving 6-7 doses.

2. Why are remedesvir so important ? Why doctors are prescribing only that dose ? Isn’t there some other alternative or other dose which is somewhat similar ?
Dr. Reddy’s Laboratories launched generic drug Remdesivir under the brand name Redyx in India on September 9.

If you're a healthcare provider, whether you're a nurse, therapist, doctor, counselor, etc... If you laugh at your patients, then with all due respect, you can go fuck yourself.
I don't care how ridiculous you think your patient's hypothesis is, or how important you think you are.

You don't get to laugh at the people for whom you're providing healthcare. Have some fucking respect for your patients.
Imagine dealing with a problem until it becomes unbearable, looking for solutions on your own, then finally asking for help (and possibly paying 100s of dollars to do so).

Then, the person you ask for help laughs at you and/or tells you that you're exaggerating.

We #doctors are taught to believe that the solution to the so-called #obesity epidemic is to tell our patients to lose weight. I think that's the WORSE thing we can do. Telling patients to lose weight usually has the complete opposite effect longterm. Wanna know why?
There are a number of factors responsible for weight gain. Diet and exercise (obviously). Stress. Genetics. Hormones. Age. Medical conditions. Certain medications. Level of education. Income. There's a wealth of evidence that demonstrates weight gain is not just about lifestyle
But in most cases most of these factors cannot be avoided. We have no control over our patients genetics, whether they have had children, are going through the menopause, or have certain medical conditions. All we can do is try to help them with their modifiable risk factors.

The first practice I was at in the US was a 2 doctor office. I worked in one half with 2 assistants and the other dentist in the other half.

One of my assistants had been in the business for over 30 years. She knew her s**t. When my colleague relocated, I asked the
office manager to have him assist the new guy because he was very fresh and inexperienced.

One day, I was in the middle of a tough root canal when she came running and said "Dr.G stat" and ran back.

I literally stopped what I was doing, told the assistant to stay with my
patient and ran over to the other side. One look at the patient and I knew he was having a heart attack.

I yelled out "911" which is the emergency helpline, asked them to say "suspected heart attack" which would get the paramedics on their way.

We got the patient on
