PSA to anyone in FL experiencing a miscarriage:
@Publix is NOT a safe place for your misoprostol prescription if you elect for medical management of your miscarriage.

1/
They will delay your care and refuse to fill your prescription until they can “clarify” the indication for your prescription even when it clearly states the diagnosis is a “spontaneous abortion,” which, by definition, is a miscarriage.

2/
Apparently, pharmacists were directed by @Publix administration to clarify all prescriptions that have the term “abortion” in them even if it already specifies “spontaneous,” and they will read you the fucking FL statute over the phone.

PSA: I am planning to reduce the activity on this account.

I'll explain why.

THREAD
Two days ago I released a bombshell tweet exposing the shenanigans that the pharma companies and academia play in order to suppress toxicity data related to novel therapies.

This is the tweet, with the hashtag #placentagate
The #placentagate tweet was then completely suppressed by twitter in a new shadowbanning technique that rendered the tweet invisible even to people who tried to click on it.

There was something special about this, so we dubbed it #placentagategate

Cheesy Debunk 🧀🧀🧀...

HUGE red flags over this #pregnancy and #miscarriage paper, which is absolute junk.

We're going to dive in and show you why this should never have been published and anybody associated with it will be forever tainted.
academic.oup.com/humrep/advance…
Here's the first red flag.

Redacted tables. I have never seen this in more than 20 years reviewing papers.

This is from the actual pdf on the Hum Reprod page for the paper.
web.archive.org/web/2023022600…
Now we're going to have to assume (because the titles are redacted🤦‍♀️) that the first forest plot shows the #miscarriage rate in each study. The bigger the square the more the weighting in the study - generally more for bigger studies.

The two largest raise a massive red flag

Guideline summary for #ED docs on early pregnancy loss/miscarriage:
acog.org/clinical/clini…
10% of all #pregnancies end in #miscarriage, though the rate approaches 80% by maternal age 45yrs.
Half of early pregnancy loss (AKA miscarriages) are due to chromosomal abnormalities. 🧵
Crown-rump length (CRL) 7mm or greater and no heartbeat. Mean sac diameter (MSD) 25mm and no embryo.
Each one is an #ultrasound feature diagnostic of early pregnancy loss.
#POCUS
#medtwitter
80% of patients with early pregnancy loss can be safely managed with an expectant management (watch and wait) approach.
Relatively few miscarriages will require ED care; most of these will involve significant and symptomatic haemorrhage or infection.

Female physicians have higher infertility rates and pregnancy complications

1. Female physicians, particularly surgeons, have a significantly greater incidence of #miscarriage, #infertility, and #pregnancy complications compared to the general population
doi.org/10.1097/sla.00…
2. In another study, of 692 female surgeons, 290 (42.0%) had a pregnancy loss, more than twice the rate of the general population.
*Female surgeons operating 12 or more hours per week during the last trimester of pregnancy were at higher risk of major pregnancy complications
3. Female surgeons had fewer children and were more likely to delay having children because of surgical training.
Incidence of postpartum depression and musculoskeletal problems were also higher in them

This @NEJM paper did long-term follow-up on 3958 #pregnant ppl who received a #COVID19 #vaccine.

👉🏻 No increased risk of pregnancy-specific adverse events (including #miscarriage).

So why are ppl claiming it shows a 24x increased risk of miscarriage? 🧵

10.1056/NEJMoa2104983
The rate of miscarriages reported in the paper is 12.6%.

But is that higher than we would normally see?

No... 2/
The normal range quoted in the paper, based on these references, is 10 - 26%.

The range is large because lots of things affect miscarriage rate, so it varies a lot between studies.

But note the rate in the vaccinated population is within this normal range. 3/

About your partners' Rhesus factor

Rh-♂ + Rh-♀ =✅
Rh+♂ + Rh-♀=⚠️ Caution when pregnant!

Rhesus incompatibility between a woman♀ & her man♂ can cause baby jaundice, repeated miscarriages, stillbirths & more.

Humans belong to blood groups 'A', 'B', 'AB', or 'O'.

Also, we're either
- Rhesus positive (Rh+), or
- Rhesus negative (Rh-)

The "+" or "-" sign usually attached to your blood group represents your rhesus status!
3/

Being Rh+ or Rh- depends on you having certain proteins called #RhesusAntigens on the surface of your red blood cells.

If you have the rhesus antigens, you're Rh+

If you don't have the rhesus antiges, you're Rh-

@SandsUK work hard to support bereaved parents - mums, dads, and non binary parents. Lesbian mums, straight couples, single parents, wider families. Anyone who has had a baby that died who needs advice and care. Please donate to them if you can afford it sands.org.uk 🦋
When you’re writing about #miscarriage #EctopicPregnancy and #stillbirth language matters. But it is not as straightforward as it might seem. If you’re talking about “mother” as the person who’s physically been pregnant and is straight it’s clear who you mean by “bereaved mum”
But if you’re wanting to make lesbian and bi mums feel included in support and care, just as we should do with dads, it can be confusing if you use the term “mother” to mean the person who was pregnant. There are two mums, one was pregnant and had the physical experience of loss

#Miscarriage is the most common reason for losing a baby during pregnancy.

Losing a baby in pregnancy is still a taboo subject worldwide, linked to stigma and shame.

It’s every parent’s greatest fear and we need to talk about it.

🧸 bit.ly/2xRbgcZ
Pregnancy loss is defined differently around the world, but in general:
#Miscarriage = A baby who dies before 28 weeks of pregnancy
#Stillbirth = A baby who dies at or after 28 weeks of pregnancy

🧸 bit.ly/2xRbgcZ
There are many reasons why a #miscarriage may happen, including:
• fetal abnormalities
• the age of the mother
• infections - many of which are preventable such as malaria and syphilis
Pinpointing the exact reason is often challenging

Sure, "polls aren't votes", but 15 pts down means Trump's (ym"sh) vulnerable, dries up fundraising, allows GOP electeds to contradict him publicly, allows reporters to treat him normally & not as the magician of 2016 who commands a cult of violent kooks.
To combat the complacency that the polling imbalance could bring, I suggest telling fellow anti-Trumpites that no matter how much DJT is down in November, when you vote, it will feel as good as punching him in the face with all of your strength

Happy #MothersDay!

We celebrate all mothers out there who brought new lives into this world.
Improving maternal & child health is a WHO priority, today and every day.
Every day, approximately more than 800 women die from preventable causes related to pregnancy and childbirth bit.ly/2LeQt5D

Preventing deaths and complications from preterm birth starts with a healthy pregnancy.
WHO recommends antenatal care that includes a minimum of 8 contacts with health professionals throughout pregnancy to identify and manage risks bit.ly/2A0gujJ

1/3 Today I share my #childlessnotbychoice story at @linsdrabwell childfree blog. I am nowhere near to saying I am childfree, but I am now comfortable with saying I am childless not choice and that being #childless doesn't define me. I am more than what I am not. Image

@RThienprayoon's tweet about this award resonated with me, though perhaps for different reasons. Like her, I had a tough early career step in #hpm #hapc. The first program chief my hospital had ever had, but isolated from a collegial network and with little local support. 1/7
Add in a late start to fellowship, and then 2 years of #miscarriage, #recurrentpregnancyloss and secondary #infertility. Those experiences made me a much better person/doctor/leader/human being, but let's be honest, weren't great for my early career leadership development. 2/7
Early #workingmomlife was a blur of hard work, sleep deprivation, disenfranchised grief, and lessons learned. The transformation was happening, but I couldn't see it. Not until I landed in a more supportive environment. #womeninmedicine 3/7
