So, lots about @WHO calling an end to the #PHEIC
First, this is a response, and it has a defined set of criteria. Whether or not these are met doesn't actually mean that #COVID isn't a pandemic, it's an interpretation of guidance and is pretty arbitrary, really.
Second, and this
really IS important...whether or not the WHO, mainstream/social media, or any other form of messaging put this across properly, national governments will have been briefed IN FULL, AHEAD of time, with the five pillars of the ongoing response in black and white, LARGE FONT etc...
Third, that certain Governments refuse to heed these recommendations, or even bother to review their own policies on how they're handling things is down to them, NOBODY else.
My opinion, fwiw, is that this is an abdication of public health responsibility that, as usual, affects

So #Covid19 is no longer a Public Health Event of International Concern #PHEIC . It is important to understand this DOES NOT mean it has stopped spreading or that it no longer represents a #publichealth risk or that covid related burden of disease is over. A quick explainer : 1/6
There are 3 criteria for @WHO to declare a #pheic according to the 2005 International Health Regulation: 1) an extraordinary event, 2) a public health risk to other States through the international spread, and 3) potentially requires a coordinated international response 2/6
All three criteria need to be fulfilled- once declared it triggers a specific response level globally, regionally and nationally. The status is reviewed regularly 3/6

'Each peak of a ‘wave’ (a new epidemic).'
❔Likely that new COVID-19 waves, and hospitalisations, are driven and dominated by a genetically different variant of the virus called #SARSCoV2.
'𝗩𝗶𝗿𝘂𝘀𝗲𝘀 adapting to the situation, through selection and expansion of 𝘃𝗮𝗿𝗶𝗮𝗻𝘁𝘀' - in response to our immune system’s pressure on their evolution - has been the one constant driver of every wave in 🇦🇺, and probably everywhere...' @MackayIM

OK, so @WHO have, in the space of a few weeks, confirmed that SARS2 remains a #PHEIC, then compared it to seasonal influenza...
Anyone else confused/astonished/outraged?
No, we are not in the same position as in 2020, and thank goodness that’s the case. But, it does not mean the
job is done. Consider if COVID emerged now as a new disease with associated harms, would we act? Of course we would.
Ironically, the fatigue many feel is due to mismanagement of the pandemic and swinging like pendulums from extreme measures to the total absence of restrictions.
This does not occur in many South East Asian countries, who brilliantly applied test, trace, support and isolate both prior to, and following the availability of vaccines.
I do not understand how a virus that continues to cause mass hospitalisation, longer term and latent health

On Friday the @WHO's Emergency Committee on #Covid19 met for the 14th time. Their report was just published and they recommend keeping #covid19 as a public health emergency of international concern (#PHEIC) for now:
who.int/news/item/30-0…
@WHO But they also say "that the COVID-19 pandemic may be approaching an inflexion point" and recommend that WHO and partners "should develop a proposal for alternative mechanisms to maintain the global and national focus on COVID-19 after the PHEIC is terminated"
@WHO "The Committee also requested the WHO Secretariat to provide an assessment regarding the regulatory implications for developing and authorizing vaccines, diagnostics, and therapeutics if the PHEIC were to be terminated in the coming months."

On Saturday, @WHO Director-General @DrTedros declared the global #monkeypox outbreak a public health emergency of international concern—what’s known as a #PHEIC.

What this means and what you need to know ↓
A #PHEIC is a global alert and call to action in response to an international health risk.

PHEICs have previously been declared for other disease outbreaks, including #COVID19, Zika, #polio, Ebola, and H1N1.

Watch @DrTedros' remarks:
Before the declaration, the DG considered factors incl. current info about the outbreak, expert recommendations, & global risk analysis.

@WHO also provided temporary recommendations to help control the spread of #monkeypox.

BREAKING—WHO committee has voted 6 “Yes” and 8 “No” for declaring #monkeypox a global public health emergency. But WHO Director Tedros might declare PHEIC anyway—he has the final say. Please do the right thing @DrTedros!
Let’s act to declare #monkeypox a global emergency now @DrTedros! Thank you. #PHEIC
3) UPDATE: Sources inside the WHO say that YES votes for a PHEIC were mostly by those with expertise in #monkeypox, LGBT health, and those voting NO against by non-monkeypox expert members of the committee. 🤦🏻‍♂️

1. A #MPXV 🧵
The director of @WHO_Europe issued a stark warning today about the state of the #monkeypox outbreak. Case counts have tripled in Europe in the past 2 weeks, @hans_kluge said.
who.int/europe/news/it…
2. @hans_kluge also noted while the majority of cases are still men who have sex with men, infections are being reported outside of that demographic. There is no reason #monkeypox will restrict itself to spread within a single group. Given the opportunity to transmit, it will.
3. @hans_kluge appeared to predict the #monkeypox outbreak will be declared a public health emergency of international concern sometime soon. An expert committee met last week & advised against declaring a #PHEIC for now.

“In many ways, women have suffered disproportionately from the pandemic”, says @DrTedros at @WHO #covid19 presser on #InternationalWomensDay. "We have seen appalling increases in violence against women and reduced access to services for sexual and reproductive health."
@DrTedros @WHO “In relative terms, employment losses have been higher for women than for men”, sas @DrTedros. "Women have also borne an additional and disproportionate burden of care for children and older people.
But women have also been at the forefront of the response."
@DrTedros @WHO "About 70% of all health workers globally are women and they have played a key role in delivering care and saving lives”, says @DrTedros. “But although they make up the majority of the global health workforce, women only hold 25% of leadership roles in health."

“This will not be the last pandemic”, says @DrTedros at the @WHO presser on #COVID19. “When the next pandemic comes, the world must be ready - more ready than it was this time."
@DrTedros @WHO Many countries have made huge advances in medicine, says @DrTedros. “But too many have neglected their basic public health systems, which are the foundation for responding to infectious disease outbreaks.” Building back better must mean investing in public health, he says.
@DrTedros @WHO Running through several countries that have responded well, Thailand, Uruguay, Mongolia, Mauritius and more, @DrTedros says: "Many of these countries have done well because they learnt lessons from previous outbreaks of SARS, MERS, measles, polio, Ebola, flu and other diseases."

Hmm. "22nd January" seems remarkably early, especially if one calibrates against what we now know happened that day.

Here's the minutes of the "precautionary #SAGE" meeting on 22/1/20:
gov.uk/government/pub…

"DHSC and PHE also preparing plans... for the follow up of contacts" Image
...and the @WHO meeting held on 22-23 January decided *against* declaring #2019nCoV a 'Public Health Emergency of International Concern':
who.int/news-room/deta…

(The #PHEIC wasn't declared until 30 Jan.)

So why were @PHE_uk officials calling @SercoGroup about #CallCentres?
We know #Serco's initial £45.75m #CallCentre contract was awarded (by @DWP) on 20 March: contractsfinder.service.gov.uk/Notice/0b32fef… but there's no mention of them attending Dominic Cummings' #BigTech meeting at Number 10 earlier that month:

“The pandemic does not mean life has to stop”, says @DrTedros at @WHO presser. "We must all learn to live with the virus, and to take the steps necessary to live our lives while protecting ourselves and others, especially those at the highest risk of #COVID19."

@DrTedros @WHO But young people are at risk too, says @DrTedros. “One of the challenges we face, is convincing younger people of this risk. Evidence suggests that spikes of cases in some countries are being driven in part by younger people letting down their guard."

As #Brazil becomes the new epicentre of #COVID19 these are my must reads to understand the political context in which the outbreak emerged and the impact of this (a thread)
This piece by @Deisy_Ventura early on in the outbreak captures a lot of the anticipated tensions within the SUS (unified health system); between Bolsonaro, science and populism (similar in many ways to US, UK etc) - which unfortunately have become real!

americas.chathamhouse.org/article/betwee…
Equally great background to #Bolsonaro role in driving the #COVID19 spread in #Brazil - and how much of this is down to politics over public health:

#covid19 presser @WHO starts with @DrTedros ceding the floor to Didier Houssin, who chairs the emergency committee on #covid19. The committe met again yesterday, 3 months after recommending a #PHEIC be declared.
@WHO @DrTedros Houssin says several recommendations were made to @WHO, highlights a few:
- mitigate possible disruptions of food supply in some countries
- develop strategy guidance for a safe return to normal passenger travel
- revise recommendations on appropriate travel measures
@WHO @DrTedros Also recommendations to member states:
- support WHO leadership "We have only one WHO and we are in the middle of a pandemic”
- work with WHO and other partners to interrupt virus transmission
- address knowledge gaps. “We know too little about the transmissions of the virus”

#covid19 presser @WHO starts with @DrTedros saying almost 2,5 million cases have now been reported to WHO and more than 160,000 deaths. "We see different trends in different regions and even within regions”, he says.
@WHO @DrTedros "Most of the epidemics in Western Europe appear to be stable or declining”, he says. But says there are "worrying upward trends in Africa, Central and South America and Eastern Europe”.
@WHO @DrTedros "Most countries are still in the early stages of their epidemics”, says @DrTedros. In other places that were affected early by #covid19, there is a resurgence in cases. "Make no mistake: we have a long way to go. This virus will be with us for a long time."

Tomorrow will mark 100 days since WHO was notified of first cases of “pneumonia with unknown cause" in China, @drtedros says at start of #Covid19 presser @WHO. “It’s incredible to reflect on how dramatically the world has changed, in such a short period of time."
@DrTedros @WHO Gives a long run-down of everything @WHO has done in those 100 days. “We have kept the world informed about the latest data.” (Hard not to feel that this is WHO justifying its actions after Trump’s criticism yesterday.)
@DrTedros @WHO Highlights:
5.1. - notified all Member States of this outbreak
22.1. - convened emergency committee
30.1. - reconvened committee and declared #PHEIC (at that point 98 cases and no deaths outside China)
February - international expert team visited China
