Dr Bridget Carne - Consultation Skills

What are they?

Communication skills
- creating a relationship
- sharing information
- picking up cues
- working out a plan together

#SWPinPC
“Consultation skills is a massive subject, so let’s focus on some some tools”

Patients often rehearse what they want to say - try introducing yourself and NOT asking a question - give the patient time to speak first.

#SWPiPC
Practice the golden minute.

Try and keep silent for 1 minute and let the patient tell you what they want to tell you - the patient will often tell you what need to know.

Try active listening (nods, yeses, mmhm etc) to encourage the patient to speak.

#Ambulance staff & #Paramedics are you feeling anxious about impending events? Here are some top tips to get you through.

A short thread.... 🧵

1/5
1. Know that you ARE doing the best you can - be compassionate towards yourself.

2. Understand what is and isn’t in your gift to control - whilst things are horrendously challenging, many events you are facing at present are beyond your control.

2/5
3. Focus on the things within your control - that patient in front of you right now, things that you can do to give yourself a bit of respite - even if it’s one small thing.

4. Limit caffeine and sugar intake, which can make anxiety worse and make it harder to get to sleep. 3/5

Last week @Ldn_Ambulance and @KingsMaternity came together for some joint maternity training. Some of the learning may be useful for #communitymidwives and #ambulanceclinicians in London so I am writing up a thread 🧵
🧵 3rd stage of labour
Midwives' scope of practice: MWs sometimes give an oxytocic drug to aid birth of the placenta (active management)
This will be syntometrine or syntocinon depending on area/trust
This is followed by controlled cord traction (CCT)
Normally takes 5-30 mins
🧵 3rd stage of labour
Ambulance clinicians can instead use physiology to encourage this process (esp if MW not present)
- Encourage bladder emptying
- Use gravity/upright positioning (have something ready to catch the placenta!)
- Oxytocin helps! (Skin-to-skin, breastfeeding)

1/ It's extremely important for registrants (especially #paramedics) to participate in this HCPC consultation. No person with their profession & good character at stake should be forced to have a #FitnessToPractise hearing by Zoom. Remote hearings should only occur by consent.
2/ All studies on remote hearings have noted their significant disadvantages by comparison to in-person hearings.
"We must acknowledge that for many people and in many cases they are not going to provide the best form of hearing we can offer" - Sir Keith Lindblom
3/ I'm concerned that the HCPC has vehemently opposed all requests for in-person hearings since Spring 2020 & has (prior to this consultation even launching) relinquished much of its London hearing space. Please participate in the consultation & try to protect registrants' rights

With an increasing number of #specialist & #advancedpractice roles emerging for #paramedics, I thought it might be useful to assimilate some thoughts on preparing yourself & your CV for these positions.
For me, it’s about a rounded approach. Undertaking every course under the sun is fine but won’t provide you with the breadth of development required. Consider the four pillars of advanced practice. This document from HEE is a useful guide hee.nhs.uk/sites/default/…

A New Year’s Day plea on behalf of all the UKs Therapists who want to help rebuild our nation from this tragic pandemic

We need investment across the pathway - we haven’t seen a penny

#LongCOVID clinics are a start but no where near enough 1/6
We have the skills and ability to care for people with complex rehabilitation needs

We work in multi-disciplinary teams, autonomously and in professional groups from intensive care to community

There isn’t a space in health and social care that you won’t find us 2/6
Yet still we suffer from lack of any serious investment. Often being the after thought to business cases, and being expected to do more with less

We have yet to see serious commitment to rehabilitation pathways & if it can’t come now, during a global pandemic, when will it? 3/6

Thread⬇️: I spent some time documenting the brave men and women of Cruz Roja, the Mexican Red Cross, in Tijuana, Mexico, who are risking their lives to help the sick during this pandemic. #coronavirus #COVIDー19

latimes.com/california/sto…
The coronavirus is killing so many people in Tijuana that the morgue has run out of refrigerator space for bodies. On the front lines of the pandemic in Tijuana is the Mexican Red Cross. Its 13 ambulances handle the majority of emergency calls for the city of 1.8 million people.
Even before the #pandemic arrived, this sprawling town was dealing with a high murder rate. The pandemic has effectively increased the case loads and risk for Red Cross paramedics, on top of the response to violent crimes and other emergencies.

Named for the crown-like spikes on their surface. Coronaviruses are zoonotic, meaning they are transmitted between animals and people. Like MERS and SARS, #COVID19 is an infection of the respiratory system.

First identified in Wuhan, China in Dec 2019. #COVID19 has reached 169 countries, with ~235,000 cases and ~10,000 deaths. And it is spreading quickly. It took over 3 months to reach the first 100,000 cases, and only 12 days to reach the next 100,000.

MDA teams took samples from 942 people who are quarantined; yesterday, more than 200 additional senior medics in MDA were trained to take samples.
Yesterday, MDA stations throughout the country held training sessions on how to treat patients, and take samples of those who are suspected of contracting the #Coronavirus.
They practiced taking samples from the patient while wearing full anti-contamination suits, and they have joined the 200 paramedics who have already started to carry out the testing last week.
