I feel the concern healthcare providers, especially those helping kids are feeling right now. I feel it both as a previous ER physician and a father of two little kids. I can feel the fear of health care providers right now.
I’ve also worked in healthcare in other countries where I had to triage for inadequate care; I’ve had children die on me, because their health system failed them. It’s not something you ever fully recover from, and it’s something we must do everything in our power to avoid here.
That means taking prevention of current infections seriously. Now. It means getting our kids, but all of us up-to-date & protected by vaccines (COVID+Flu). It means masking up at school, work, and out in the community, anywhere we can spread. It means staying home when sick!

📊 Ontario Science table’s survey shows significantly lower vaccination intention amongst groups who experience a disproportionate burden of COVID outcomes: low income & racialized.

As someone who belongs to this group & does literacy outreach I’d like to dissect it further 🧵⤵️

This phenomenon isn’t new. In every aspect of medicine, racialized, marginalized, underserved & low-income groups, who often experience the highest burden of bad outcome, also happen to be the most reluctant to seek medical intervention, including vaccination. But why⁉️

Barriers to access is at the root of the issue‼️

These barriers include:
📌 Mistrust
📌 Low health literacy
📌 Financial & logistical burden

Canada has reported 26,688 COVID-related deaths - 13th highest amongst the 38 OECD nations, though our experience has been less intense than other G7 nations.

But are we reporting deaths coherently?

No.

Were lessons learned from previous waves to change this?

Also no.

1/I have to believe this decision wasn't made lightly.

What I need to hear (and will actively support):

Resources will be committed to develop an integrated strategy for in-person instruction as a healthy, resilient workplace in Sept 2021

Why this is so important:
2/Schools offer an essential service to students

Many have benefitted from in-person instruction and stable access to clinical services as needed.

And more have benefitted when there have been additional educational supports (e.g. EAs)
We have seen investments for schools to be healthier workplaces for students

and for the education staff who are critical to their essential work.

But these measures haven't been consistently applied across communities, despite PHUs, education admin, staff, parents leaning in.

1. Workers are ineligible for CRSB the week they receive WIPB. CRSB meant to complement paid sick days whereas this policy makes them ineligible. Workers already struggling now have to deal with granular details of multiple applications & still end up with financial loss 2/4

So here's why I'm concerned. We are seeing VOCs absolutely explode in ON, and at the same time the test positivity is going back up, so the cases are probably outpacing our testing strategies.
So much of what was said in the ON press conference earlier this week made it clear the focus is on vaccines. That's awesome, I am very pro-vaccine. But here we are on Friday and folks in hot spots still don't know how or *if* they can get a vaccine. toronto.citynews.ca/2021/04/08/ont…
So this means that we can't be relying 100% on vaccines right now to solve this exploding VOC problem. To me this means:
-We need to seriously consider closing schools throughout ON
-We need to make it VERY EASY for folks to not come to work sick (#paidsickleave)

1/It's the eve of provincial announcements on schools reopening for in-person instruction.

Households are under stress and experts are divided on whether schools are unicorns or infernos.

Everyone wants to do right by kids, who have borne so much throughout this pandemic.
2/As @AmyGreerKalisz, @AshTuite and I wrote in July, the most effective strategy in schools is to decrease community transmission.

Been taking a closer look at the #paidsickdays and #familyleave provisions of the #familiesfirst Coronavirus Response Act that the House just passed. Some first impressions.
The bill amends the #FMLA AND has a new Act that requires private employers with fewer than 500 workers and all public employers to offer #paidsickleave.
Mandatory #paidsickleave ONLY applies if you are affected by #coronavirus and need time off because you are sick or exposed or if you have a family member who is affected (either sick or home from school or childcare because of closures).

The richest country on earth & in the history of humankind should be better prepared for the #CoronavirusOutbreak. Here are some reasons we’re not:

1. No #MedicareForAll means patients face financial hurdles (co-pays, deductibles, other bills) that deter them from health care...
2. Financial hurdles, unnecessary paperwork, & pointless scrutiny of immigration status when determining health care make it harder for people of color & low-income families to access medical services. #COVID19 will hurt these communities harder because of these unjust processes.
3. We don’t have #PaidSickLeave to allow sick patients the time & financial security to recover. Most of us go to work sick, not because we’re gross or cruel, but because our livelihoods are on the line. Those of us with sick days still go to work cuz our bosses expect them to.
