Distraught parents bring their son to you. They tell you that he has been having weakness of his left hand since 1 year. With wasting. No sensory symptoms. They have been told, Motor Neurone Disease . You look at his hand and smile reassuringly. Why?
First things first, the demographics don't fit ALS. He's a young boy. Secondly, The disease seems quite restricted. Third, no signs of upper motor neuron disease. Finally, this OBLIQUE AMYOTRPOHY is classical and clinches a diagnosis of HIRAYAMA DISEASE

#MedTwitter #NeuroTwitter
Hirayama Disease typically affects young Asian males in their twenties. I have seen dozens of patients with Hirayama, never a girl (although case reports exist)
These boys tend to be lean and have long necks!
It is much more benign compared to ALS!

HYPOTHALAMUS (HT)🧵-the control center of circadian rhythm, fatigue/wakefulness, hunger/satiety, sex drive, thirst/BP—and the command post for endocrine control via the HT-pituitary axis.

Quiz A: Histamine activity in the brain/brainstem contributes to alertness/wakefulness–hence: sleepy effects of benadryl. Histaminergic neurosecretory cell bodies are exclusively in the tuberomammillary nucleus of the TUBER CINEREUM (HT floor), with widespread projections.
The HT is a mysterious and complex—I get confused sighs from medical students when the topic arises—the small almond-sized morsel is the control center for endocrine/hormone regulation, and homeostasis of food/water consumption, sleep, BP, sex/attachment. The HT is boss!

1/11 A few years ago, I took care of two young women (some details changed) with similar scans… a brief thread about how MRI can shape a Ddx… but first a poll: Which lesions can restrict diffusion?
2/ Yes… all of these lesions can restrict diffusion. For more info on restricted diffusion see tweets 9-18 in this wonderful tweetorial from @teachplaygrub:
3/So some brief history about the 2 pts. First patient presented to another hospital months ago with R-sided hemiparesis and dysarthria. CSF showed elevated protein but no pleocytosis. Treated several times with steroids with transient improvement but has clinically progressed. Image

Dear #hr in #niin #ksom #usc, I know you are busy, but could you spend 8 mins on my emails pls? I sent 6 emails from April to June, and called you every day this week, but I didn’t get any reply. #neuroscience EAD will be expired at 07/10😩@USC @KeckMedUSC @KECKSchool_USC @USCNIIN ImageImageImageImage
Thanks, friends in #twitter in #niin,#ksom, #usc, in #neuroscience, #neuroimaging, #neuroradiology. I really appreciate your support. I got the email this afternoon. It’s my fault. Even leave, leave with pride, grace, and gratitude, no regret. The 4 yrs will be the best memory in Image
my life, although it is also the hardest 4 yrs. Hope other colleagues could take my lessons, and prepare everything earlier in the future.

Everyone loves those pretty colored fMRI pictures with the blobs. But what do they mean? How do they make them?
A #tweetorial called “F— that: Understanding fMRI”. #FOAMed #medtwitter #Medstudenttwitter #neurotwitter #FOAMrad #neuroradiology #radiology
fMRI is based on a principle called “neurovascular coupling.” This is the principle if there is increased neuronal activity in a region, there will be increased blood flow to that region to meet the increased demand
Think of it like a baby crying because it is hungry—parents immediately rush to feed it. The increased oxygen demand of the neurons immediately brings increased fuel to feed it.

re: ICH Prevention in People w ⬆️Risk MRI findings in @American_Heart updated 2022 ICH guidelines🧠🩸

📌 Refers to incidentally found MRI markers of small vessel disease (SVD) - a common question

Prompted by @interneurona unpacking below (short 🪀)
For context:
🚫MRI is not routinely done for risk stratification of first-ever spontaneous ICH risk

🔮MRI is occasionally available in certain people/pts w/o ICH: may show SVD markers concerning for future ICH risk

〽️These markers are: microbleeds, superficial siderosis

The populations that this might be relevant to are:
- healthy elderly
- ischemic stroke
- memory clinic
- any pt who got a brain MRI for a different indication (e.g. migraines)
- non-ICH cerebral amyloid angiopathy (CAA)

CCA - differentiation of reactive lesions from progressive ones following cranial radiotherapy - a thread 1/n

➡️ primary & secondary brain tumors (glioblastoma/brain mets)
➡️ after high dose radiotherapy
➡️ N=33, prospective confirmatory study…
➡️unclear findings on conventional MRI (psPD/RN vs PD)
➡️contrast clearance analysis with 1 h delayed CE MRI, images were substracted from each other
➡️ histopath confirmation
➡️ overall accuracy very high (84%)
➡️ subgroup of secondary tumors with high benefit (accuracy 94%!)
Therefore a potential new standard? Amino acid PET in a smaller subgroup did not outperform CCA @galldiks , perfusion/DWI more prone to institutional deviations/less potential to be standardized? @gminniti2012
