My mom was hospitalized with low O2 (82) and inability to standup due to fainting (hypoxia) due to #coronavirus on Day 8 of her infection last Sunday, 12/20.

She received #Ivermectin and Dexa and a mild steroid. She received 2nd round of #Ivermectin Tues.

Symptoms ~70% better
Since #Ivermectin, She has recovered her sense of taste and smell.

No more rigor (shaking) due to probable #coronavirus sepsis.
Fever reduced to 101 F and only @ night.
Cough reduced 90%.
No more sore throat.
Blood O2 @ 92-97 [safe bounds, avg 95], checked hourly.
She took it way too late (Day 8 and 10).

For best results take #Ivermectin on exposure or at FIRST SIGN of drainage or throat tickling...

DO NOT WAIT til you're collapsing like my mom did.

#Leronlimab is an antibody that blocks a cell receptor called CCR5. The #CCR5 receptor just happens to be a key doorway HIV uses to get inside a cell so that’s why we developed it as an HIV treatment. But the actual function of CCR5 inside our body is to direct cell movement and
Function. Breast, prostate, and a few other cancers co-opt the immune system to support their growth by using the CCR5 receptor to control cell movement and function. We were targeting that role of CCR5 to support cancer cell growth when COVID-19 hit. Our early signals of success
in breast cancer led us to believe we could impact the inflammation, or #cytokinestorm, that is killing patients with Covid-19 since it is the same cell movement and function controlled by CCR5 that is involved. And, as incredible as it sounds, I believe we got very lucky and

(1/3) Timeline of a #covidiot.

37 yro Richard Rose of Ohio said he thought #coronavirus was "a hoax" and was very pro-Trump and very strong anti-masker, like @SarahConner39.

April 28th: he said this: Image
(2/3) July 1st he became symptomatic and got tested with a rapid test. Confirmed #coronavirus positive.

On 2 July 2020, he posted this: Image
(3/3) The next day, July 3rd he posted this.

But by July 4th, he was *DEAD*! #CardiacArrest, probably from #CytokineStorm.

This #covidiot learned the hard way that #coronavirus is NOT A JOKE!

Subject: Critical appraisal of Itolizumab in #Coronavirus infection.
Events: 1. @sardesairajdeep ran a programme on 11 July showcasing what was claimed to be a breakthrough in the hunt for a drug that worked against #Covid19. Featured #Biocon's @Kiranshaw
Kiran Mazumdar-Shaw has blocked me so she won't see this thread. You can watch @sardesairajdeep programme here: .
Essentially it explained the #cytokinestorm that happens is some patients with #Covid19 and can be fatal. #Itolizumab counters this.
What is Itolizumab? Here is it's Wikipedia entry: It is a relatively newly developed drug in a class of drugs known as monoclonal antibodies (all of them end in '-mab'). They have been around as a class of pharmacologic agent since 1986.

7 Reasons Why @challiance is Giving N-acetylcysteine (#NAC) to #COVID19 + Patients with Early Respiratory Symptoms

NACinCOVID.net
#NACinCOVID
@MLaiB Image
1. #NAC is a powerful antioxidant...
NAC=N-acetyl derivative of amino acid L-cysteine, precursor of the body’s primary intracellular antioxidant glutathione (GSH). (Behold the reducing power of thiol -SH groups!)
... antioxidant #NAC scavenges reactive O2 species (ROS) that signal pro-inflammatory molecules that can incite #CytokineStorm

... hope is to prevent body's immune response from overshooting and progressing to cytokine storm and #ARDS

(1/7) Q. Is obesity really a risk factor for #COVID19?

A. It’s not necessarily a risk factor for infection, but a small number of studies suggest individuals with BMI > 30 (defined as "obese") are more likely to be hospitalized than individuals with BMI <30.
(2/7) Other studies show that individuals considered obese are more likely to need ventilation or admission to acute and critical care units.
These patterns are especially pronounced among adults YOUNGER THAN 60/65 years of age (depending on the study)
(3/7)
Excess body fat can induce immune dysregulation and chronic inflammation which is linked to the #CytokineStorm responsible for Acute Respiratory Distress Syndrome in respiratory diseases.
Abdominal obesity, especially, can cause compress in the diaphragm and lungs.

let's talk about #CytokineStorm in #COVID19.

1st, there is no standard threshold above which something is called #CytokineStorm.

Mechanistically, the simplest form of #CytokineStorm we can understand is when anti-CD19 #CART cells are infused in tumor-bearing recipients.
in this scenario, we have large numbers of antigen-specific T cells (anti-CD19) and large numbers of antigen-bearing cells (CD19+ tumor cells). When too many T cells engage with too many antigens simultaneously in a short period of time we get #CytokineStorm
Why is that? This is because naturally developing an adaptive immune system (T cells) ordinarily doesn't work that way. 1st, for a given antigen we have very few antigen-specific T cells (~ 50 cells/antigen, unlike anti-CD19 CAR-T cells that are in millions).

Starting a series on clinical Pearls I am gathering in #COVID19 patients based on experiences of many experts

Will add as we go along

Feel free to add your own observations/experiences

#covidclinicalpearls /1
Anosmia is likely the most specific #COVID19 related symptom
30% of patients have anosmia as their 1st symptom
#covidclinicalpearls /2
Around 90% of patients have fever.
50% maybe afebrile at the time of presentation
Fever tends to be very resistant to routine measures in hospitalized patients
There is no consensus that NSAIDs are to be avoided in #COVID2019 patients

"Cytokine Storm". The idiot's buzzword of the week. It makes you sound smarter if you say it often enough. When I first heard of it in 1994 it sounded AMAZING! Then this RCT came out showing it increased mortality. Ouch nejm.org/doi/full/10.10…
Next came rhAPC (drotrecogin). Don't knock out the immune system: we need it to survive. APC has signalling properties to modulate inflammation. NEJM PROWESS was touted a success! nejm.org/doi/full/10.10…
Just as we were celebrating and retrospective analyses showed efficacy correlated with drotrecogin reducing Il-6 levels, bad news came in and the dream turned sour nejm.org/doi/full/10.10…

After a week of self-isolation (with presumed mild COVID 🤒) reading just a few of the astonishing number of #SARSCoV2 & #COVID19 papers/pre-prints in between doses of paracetamol... Some thoughts and questions about what we know so far about COVID19 #immunology. 1/12
The big questions (IMO): does an aberrant immune response contribute to bad clinical outcomes? What is the nature of this immunopathological response? And conversely, what is a protective immune response to #SARSCoV2? 2/12
Inflammatory cytokines eg IL6, TNF correlate with severity/ARDS. IL10, chemokines also up. High admission IL6 predicts bad outcomes. thelancet.com/journals/lance…, academic.oup.com/cid/advance-ar…. What cells drive #CytokineStorm? Does it cause deterioration or is it a bystander phenotype? 3/12

Lessons from past #coronavirus & #influenza #epidemics suggest that #viral #infections can trigger acute coronary syndromes, arrhythmias, exacerbation of heart failure owing to a combination of a significant systemic inflammatory response plus localized vascular inflammation.
#COVID19 may either induce new cardiac pathologies &/or exacerbate underlying cardiovascular diseases. A large proportion of patients have underlying cardiovascular disease &/or cardiac risk factors. Factors associated with mortality include male sex, advanced age, comorbidities.
Acute cardiac injury determined by elevated high-sensitivity #troponin levels is commonly observed in severe #COVID19 cases & strongly associated with mortality. Acute respiratory distress syndrome is also strongly associated with mortality in patients with #SARSCoV2 infection
