One led by a fantastic and experienced Advanced Emergency Medical Technician. Although more experienced than many paramedics - including myself - this person has a scope of practice without intravenous medications, or advanced life support.

In the 1970s/1980s, ambulance services were simple; every town had a station, with more ambulances than the town needs.
If you called 999, one of the crews would come, take you to hospital, and then go back to their station to clean, restock, and play pool til next call. (2/15)

Whilst really easy to understand - it only works when you have more ambs than you need, and when it's busy, your two ambulances might both be 'on a job ' and if you call 999, one of the two crews in the next town will have to come - which might be 20 miles away! (3/15)

I'm a retired 3H0x1 who documented classified air operations in Iraq in 2003 for Operation IRAQI FREEDOM. A "Deployable Enlisted Historian" is a freaky USAF job because I can stop a general in his tracks during a deadly crisis to demand a briefing, e.g.:

While not a new problem, there has been increased conversation on ambulance offload delays as we weather this latest COVID surge. Systems all over the world are struggling to get ambulances back in service in a timely manner after transporting pts to the hospital.
@NEMSISTAC nationwide data shows offload times (elapsed time from the arrival of an ambulance with a pt to the ED until the ambulance is back in service) have increased and spiked at various times in 2020 & 2021 (red + purple lines) compared to 2018 & 2019 (blue + green lines).
