After all the lies, when the dust finally settles, the truths will be known. The #WH doctors have been lying for #Trump, but we knew that. How disappointing some people are. Yeah, they snatched him back from the jaws of death and then lied. 1/2
nytimes.com/2021/02/11/us/…
They snatched him back from the jaws of death by pulling out all the stops, he had #Healthcare, plenty of nurses, plenty of #PPE, #Ventilator at hand, a dozen doctors, #ExperimentalDrugs #MiracleDrugs #CelebrityPrivilage 420 thousand other Americans died. nytimes.com/2021/02/11/us/…
And then after they pulled him back from the brink of death, they allowed him to make speeches saying "I feel great, don't fear the virus, & it won't hurt you". So, that's a lousy career those dozen docs are having. Worse than the doc who #technored #MichealJackson while he died.

#polio, let's go right to the beginning to understand that vaxx did nothing to eradicate it. So what is polio? In simple terms it is the inflammation of the gray matter of the spinal cord. It can also occur in the brainstem. Image
We have been conditioned to believe that polio equals braced limbs, iron lungs etc. Actually only those infected with #bulbarpoliomyelitis, which affects the brainstem, were placed in #ironlungmachines Image
We've also been conditioned to believe that polio was wide-spread, which actually is not the case at all. Naturally existing poliovirus was a common bowel inhabitant for
millennia, always there, continuously circulating through humans,
but never causing paralysis. Image

On May 29, 2020, IISc’s Project PRAANA team successfully completed the prototyping of an indigenously developed ICU-grade #ventilator.

Video:

(1/n)
Built using a custom designed pneumatic system controlled by a microprocessor, it uses proprietary algorithms and techniques to blend air and oxygen in the desired ratio. (2/n)
It also has fine-grained control of patient-side respiratory parameters such as respiration rate, inspiration to expiration ratio, FiO2, and PEEP. It supports both invasive & non-invasive ventilation. (3/n)

. @CovidIndiaSeva I beg your kind attention here ! If we look at proportion of #Covid_19india patients on #ventilator support, it is 0.4% to 0.5%, which reflects ARDS due to Pneumonia due to #COVID19 as cause of death (COD) ( @WHO code: U07.1/2)... 1/n
So, around 150 deaths in 3000 projected deaths are actually due to #Covid_19india Epiphenomenon: People with diabetic complications, advanced cancer etc where the main COD is underlying health conditions, but #COVID19 can only be a contributing cause .. 2/n
Vast majority of #COVID19 deaths in India could be an epiphenomenal death only, considering #BCG vaccine induced trained #Immunity (Nature paper link at the end of this thread) that offers protection from severity of #Covid_19india . Why I am saying this is more important.. 3/n

THREAD. How much does treating a serious case of #COVID cost? Here’s a (partial) figure.

First, a pleasant surprise regarding #HealthInsurance - how often does that happen? My insurer, @UHC, has waived cost-sharing for #COVID19 TREATMENT (not just testing):
I logged into @UHC today and found a slew of Explanations of Benefits (EOBs) with a nominal total cost of more than $320,000 for my #COVID19 hospitalization. Bills are still coming in, so that’s not a final figure.

#coronavirus #Covid_19 #COVID #healthcare
But under “You May Owe” on the EOBs, I saw $0, $0, $0 - i.e., @UHC isn’t hitting me with any co-pays for #Covid19 treatment. I was prepared/expecting to shell out my out-of-pocket max ($6,000 in-network, $12,000 out-of-network).

Today, about a month after I came off the #ventilator and 3 weeks after my hospital discharge, I had a telemedicine consultation with a pulmonologist.

#coronavirus #Covid_19 #COVID19
I told him about my cough, a sometimes productive cough that gets worse at night, and my shortness of breath from mild exertion.

#coronavirus #Covid_19 #COVID19
The cough is not surprising and could last 2-3 months, he said. He prescribed me a steroid inhaler that should help if it’s #asthma, which it could be. (It’s no longer the #COVID19 cough at this point.)

It was a privilege to share the (virtual) stage with an amazing panel discussion of differences between PICU and MICU care by #POPCoRNet. Thanks to @DrPhilipVerhoef @DeannaMarie208 @davidshoremd @DoctorVig @MihirAtreya @ArunK_MD @PfarrMarie @annaberry29 for arranging. #FOAMcc 1/3
We covered some important adult and pediatric ICU topics including #Pain/#Anxiety/#Delirium (PAD) treatment, #COVIDairway management, #ARDS #ventilator strategy, #proning, #vasopressors, & how to perform #ACLS while wearing PPE. I've attached some relevant OnePagers. 2/3 ImageImageImage
Very thankful to the amazing international audience who contributed great comments and discussion. Bottom line: adults are just big kids and there are some great pediatric intensivists out there who are ready care for adults with #COVID thanks to #POPCoRNetwork resources. 3/3

Starting a series on clinical Pearls I am gathering in #COVID19 patients based on experiences of many experts

Will add as we go along

Feel free to add your own observations/experiences

#covidclinicalpearls /1
Anosmia is likely the most specific #COVID19 related symptom
30% of patients have anosmia as their 1st symptom
#covidclinicalpearls /2
Around 90% of patients have fever.
50% maybe afebrile at the time of presentation
Fever tends to be very resistant to routine measures in hospitalized patients
There is no consensus that NSAIDs are to be avoided in #COVID2019 patients

MYTH BUSTER: #anaesthesia & #anaesthetists are hard words to say (I’m one & have trouble!) & people are sometimes confused about what we do (even other doctors!) and that’s fair enough, but we are way more than just people who “put you to sleep” for your surgery #covid19aus 1/
It’s better to think of us as #anesthesiologists (with an american accent!) like they say in the US. We are medical specialists. We have a medical degrees & a specialist qualification, are highly trained having done continuous training until we are often at least 30 years old 2/
We assess all aspects of you before giving you an anaesthetic - Such as your health conditions, your medication, your worries/concerns about what about to happen to you, your pain requirements, & make decisions about the safest anaesthetic to give you for your surgery 3/

The initial logic behind using a single #ventilator to support #multiple #patients is compelling, and speaks to our most basic urge to rescue.

A long thread, to outline challenges that must be solved for it to work in #COVID19
There are certain situations in which it would work well – in patients with fundamentally normal lungs, whose compliance can be easily matched and kept matched, who can be deeply neuromuscularly paralyzed, and ...
for whom there is a plan for individualized ventilators to become available soon to make vent weaning/liberation possible.

(This is important: you can NOT wean multiple patients from a single ventilator, and there has to be a plan for eventual extubation)

THREAD: I was sad, but now I am angry. As clinicians, we have the enormous privilege of attending to people when they are at their worst. Death, loss, disease, events that alter the trajectory of people’s lives, and keep working, because there is always work to be done. 1/
We ease the suffering of #humanity. That is what we have been called to do. And I feel like part of that is that we often censor ourselves to protect non-medical people. But how about we start to share a tiny glimpse of our reality? (Medical terminology is in quotes) 2/
While we know that 80% of cases are mild, do you know how people die after being infected with “Severe Acute Respiratory Syndrome #Coronavirus 2” (#SARS_COV_2)?

Rather, do you know how the majority of people who are deceased as a result of the new Coronavirus, perished? 3/

1.After reading @ManMundra @bhootnath with others collecting monies for #Ventilators, got reminded of my medicine days of crisis, complexity & cost for ventilators.

This HACK-THREAD wil b bout mitigating d cost, finding hacks and simple solutions,

@PMOIndia @drharshvardhan
2. So according to feasibility study published in 2006 (onlinelibrary.wiley.com/doi/abs/10.119…) we can use ONE VENTILATOR in minimum 4 patients , in conditions like India we can also use it for as many as 9 patients. In 2017 Las Vegas shooting this technique was used.
ImageImageImage
3. #Ventilator is complex machine used for a very simple process of breathing
A patient is given mouth to mouth resuscitation & bag n mask ventilation for survival. If #India goes in a phase like #ITALY we won't suffice Ventilators. We will have to think of simple hacks (thread)
