Increasing overmedicalisation at the end of life is denying people & their families a good death.

Towards a compassionate community model: The Lancet #ValueofDeath Commission calls for a radical rethink of care for the dying & attitudes to death. hubs.li/Q0139HH30 Death and dying must be rec...
How death is managed today:
• Alienates communities
• Pushes families in countries without #UHC into poverty
• Deprioritises efforts to reduce suffering, such as #PalliativeCare
• Diverts resources
• Is costly for health services

Scientific advances have fuelled the belief death should be feared, avoided, defeated.

Centralising community involvement, partnerships, & better bereavement support will help rebalance societies' & medicine's relationship with death—an inevitable part of life. #ValueofDeath Image

Robert Yates of @ChathamHouse and @LSHTM comes to us from a sunny London! We are so lucky to have him chat to us about how palliative care can get a "decent crack of the whip" on universal health reforms. #21OPCC
@ChathamHouse @LSHTM "This is an opportunity for radical change. We should be prepared and get organised." But what is Universal Health Coverage? Important to nail down the definition. #21OPCC Simple definition here 👇 Image
@ChathamHouse @LSHTM But the longer definition makes it clear that palliative care is part of the continuum. Wherever you see UHC, we must insist on palliative care not being crowded out by curative services. #21OPCC Image

Hi @FCDOGovUK @10DowningStreet, there appears to be something missing from your #IntegratedReview, did you forget something?
PM spinning that he'll use the review to make a 'personal commitment' to restoring the 0.7% aid target doesn't cut it. The wording of the #IntegratedReview says it'll return 'when fiscal situation allows' - not a firm commitment, another promise to be broken
Even if he were to be believed, that's still a year at 0.5%. Hundreds of thousands of lives lost. Aid to Yemen and Syria cut by nearly 2/3.

It's not a necessary cut, and in any case it comes as they announce increasing spending on *more* nuclear weapons. #IntegratedReview

My presentation at the @LagHealthScheme UHC Day Celebration.

Over the last 2 decades @followlasg had experimented with many policy initiatives such as: The Free Health Scheme; the community based health insurance scheme (CBHIS) aimed at attaining #UHC #ForAGreaterLagos
The journey towards #UHC has not been without hitches. This necessitated the establishment of an all inclusive mandatory health scheme to bridge the gap

The objective of the scheme is to reduce out of pocket spend, thereby protecting the population against the financial risks
Challenge - fragmentation and low coverage;

•2% of the population of Lagos has health insurance, including fragmented NHIS programs and private health insurance.

6 months into #COVID19, our best measures of pandemic preparedness have not been correlated w/fewer deaths

In fact, nations w/higher JEE, GHSI, and #UHC scores have had *higher* death rates, even accounting for age structure & timing of 1st case

why? 1/
#COVID19 has revealed that we don't yet understand how best to measure countries' capacity to respond effectively to severe pandemic threats

CDC Rule (2017) expressly addressed this, w HHS/CDC the "payer of final resort" (despite comments that this would be unethical).

Under §70.13 @CDCDirector can authorize payment for care & treatment, although it's secondary to any state, local, or private insurance obligations.
@CDCDirector The law provides for this, and it's unethical for the US government to require individuals subjected to mandatory care & treatment to pay for it.

This also risks undermining the #Covid19 public health response: without #UHC #Medicare4All people will understandably avoid care.
The @CDCDirector’s discretion is subject to availability of appropriations. This underscores the considerations for federal emergency funding: Congress cld specifically provide add’l funds for such costs & at the same time: the #covid19 health care costs of all Americans. #M4A

@TomSteyer @lily21951 I appreciate you and I'm for Bernie right now, but you're my number two. Currently payroll taxes pay for medicare. There is no cap like social security so no unfair advantage to the rich. Medicare for all just increases this minimal tax and eliminates premiums 1/
@TomSteyer @lily21951 , copays, out of pocket, and uncovered fees. Money goes in, patients get treatment, providers get paid. The current system (your aforementioned union health plans and others) use a non-value added middleman who receives a huge rent in the form of premiums and LOA reductions 2/
