Recapping @AmCollegeGastro's #VirtualGrandRounds on #IBS! Let's begin!
🟣IBS can be diagnosed with the Rome criteria
🟣Making a positive diagnosis is very helpful to patients, many of whom have gone years without a name for their symptoms
#IBS is heterogeneous, and so are its pathogenesis and pathophysiology.
🟣Identifying underlying factors in patients can help target treatment!

1/ How neglected is Myalgic Encephalomyelitis/Chronic Fatigue Syndrome research?

@NIH funding for ME/CFS in 2017 was ~$25mill

Fair funding relative to disease burden should be ~$350mill

That means #MECFS research was short-changed by $325,000,000 (14-fold!) in just ONE YEAR
2/ Buckle up!

@CenterRes analysis found that even though the disease burden of #MECFS was double that of HIV/AIDS, #AIDS research received ~$5billion in 2017

Thats a 20-fold shortfall, amounting to ~$4,975,000,000 lost #MECFS funding in just ONE YEAR!

pubmed.ncbi.nlm.nih.gov/32568148/
3/ And, the graph below shows that #MECFS research is woefully underfunded compared to similarly burdensome diseases

Note: the vertical Y-axis scale on the graph is logarithmic

So #IBS and #pneumonia actually receive 10x MORE funding than #MECFS

#IBS & #TMJ: an interesting essay showing why these seeming separate disorders are related; with emphasis on TMJ etiology.
Relation of IBS & TMJ: The nexus is hyper-mobile joints. TMJ occurs in TM joint of face uppermost in people who chronically tilt head; oft associated w. scoliosis (common in #HSD) & d/t Masseter muscle spasm attempting to maintain normal dental occlusion.
#IBS is associated with ligament laxity of SIJ & autonomic nervous system impingement within contiguous presacral plexus, which tracts to intestine; #IBS is a #dysautonomia. These are my clinical observations and theories

1/13 Galen's @MeatHealed Story - In the Summer and Fall of 1995 I started intermittently experiencing extremely sharp abdominal pain. Over the course of several months, symptoms escalated to severe intestinal bleeding, cramping, nausea, and weight loss.
2/13 A gastroenterologist ultimately diagnosed Crohn's disease in January 1996 following the standard diagnostic procedures (barium enema, CT scan, upper/lower GI scopes). Although I asked the gastroenterologist for dietary advice at the time, he had no recommendations to offer,
3/13 ...but prescribed me several anti-inflammatory and antibiotic drugs. At the time of diagnosis, I had just turned 21 years old, and I completely rejected the notion of having to take pills 4 times a day indefinitely.

🙏Why Doctors Should Organize

Doctor-patient relationship is crucial and I wonder if a patient is aware of the hospital’s policy (not the doctor) for a visit time allotment (in minutes).

#FIBROMYALGIA CAUSATION A LINEAR EQUATION? Theory might regard “fibromyalgia” to be a label for a variety of disorders that have simply devolved from patterns of chronic widespread pain. A Twitter Thread....
Do neural pain fiber activities initiate mast cell degranulation that then cause other populations of nerves to activate? Does degranulation occur regionally to generate a specific pattern of tissue disfunction? Does ligament pain arouse mast cell degranulation?
IMO, #MCAS association w. #Fibromyalgia requires other insights. Body stress/assault, beset w. pain & marginal metabolic activity, arouses General Alarm Response (after Hans Selye & Walter Cannon) & hyper-dynamic hormonal/immunologic functions needed for organism survival.

A query was offered about my comment that ligament laxity was common to #TMJ & #IBS: “So, you would say the two are not connected other than the fact that the person has joint laxity.” My response to this comment is as follows: t.co/tNubAPSukU
In patients with only complaint of TMD (Temporal Mandibular Joint, TMJ, Disorder) there seems to be near universal generalized joint hypermobility (#HSD). TMD seems to occur mostly in women.
In persons who complain only of symptoms that prove to be #IBS, the near universal finding is sacroiliac joint (SIJ) hypermobility.

Remarkable @bmj_latest @BMJCaseReports report documenting the extreme lengths a bedbound patient went through to get a diagnosis of #POTS, #MCAS, #SIBO and #tachycardia

Her cardiologist couldnt diagnose #POTS

Her gastroenterologist couldnt diagnose #SIBO

Her immunologist couldnt diagnose #MCAS

Her neurologist referred her to a #psychiatrist!

Dr L in the buzzfeed article says humans make alpha-gal sugar, But, here’s a podcast & transcript w/Dr Commins, Prof at UNC Allergy & Immunology who was part of the discovery team in 2009 & is an expert on AG. He says humans don’t make alpha-gal sugar./1 apple.news/A-Ybbhe0lQXuD_…
Dr Commins also clarified that the 2009 discovery involved the lone star tick but that it can be caused by other ticks. His research is ongoing. I disagree w/him on 2 things:
1) I Do think anyone w/a tick bite should be tested for the allergy apprx 6 weeks after the bite.
And/2
2) The reaction does not always cause itching. I’ve found the random palm itching to be more common than torso hives, but, even more common than that is no hives. And, I agree that the allergy response, unlike other food allergies, is delayed. Dr C say apprx 4 hrs after/3
