Acute hepatitis in kids. We urge colleagues with access to samples (blood/liver tissue) to consider SARS-CoV-2 superantigen-mediated disease, potentiated by a second virus (AdV) 🧵1/ thelancet.com/journals/langa…
2/ @ArditiMd and colleagues first described a superantigen motif in SARS-CoV-2 and several groups have independently found T-cell skewing (TRBV11-2 expanded) in kids w #MISC, indicative of a superantigen-mediated inflammation.
3/ Multiple reports have reported intestinal SARS-CoV-2 persistence and leakage of superantigens into the bloodstream trigger immune activation and possibly MIS-C in susceptible individuals, @laelyonker et al

Let's talk about the mystery epidemic of #hepatitis in children. It's most likely a complication of #COVID19 but may take a while to be adjudicated as such. The syndrome has been seen in children <17, mostly unvaccinated kids <5 years. A thread.
Hepatitis is a known presentation of #MISC onlinelibrary.wiley.com/doi/10.1111/ij… & MISC is a late complication, after the acute infection. So, the fact that PCR is negative is not surprising.
Also, kids do not have as robust an antibody response as adults so may be -ve on serology too. nature.com/articles/s4159…

The importance for quick data sharing & finding the cause for unclear #hepatitis in #children is also for the right treatment. Huge difference if it needs antiviral treatment (that only makes sense when there is actually virus) or treatment for an immune-mediated pathogenesis.
Since this is ongoing since a while, I would love to see some early data, even if preliminary (biopsy findings). And yes, transparent communication on #SARSCoV2 #COVID19 serology would also help against speculation of any kind. There are aspects for & against each theory.
Since parents were re-assured by governments & MANY health professionals that catching #SARSCoV2 #COVID19 is not a big deal for children (& therefore few - no efforts to mitigate transmission), any link, however low the plausibility, deserves a thorough & transparent assessment

@WHO had geklaagd over nieuwe inzet hiervan. Image

How come #COVID19 is mostly mild in children? And why do #MISC affect mostly young people?

An interesting hypothesis by @BrodinPetter considering the trade-off between growth and immunity, while most of the studies focus on resistance rather than tolerance 1/
Tolerance to the initial infection may allow virus to stay in gut longer, leak through gut wall, and trigger T cell activation. Could a milder systemic immune response lead to viral persistence in the gut of children? 2/

How come #COVID19 is mostly mild in children? and at the same time, why do the rare hyperinflammatory syndrome #MISC affect mostly young people, and not the elderly? thread🧵and a hypothesis: cell.com/immunity/fullt… Image
2/In other infections, ex mononucleosis, symptomatic disease in teenagers differs from mild/asymptomatic infection in young children by stronger bystander T-cell activation and systemic inflammation.
3/But what limits systemic inflammatory responses in young children? One likely explanation is that energy allocation for growth increases the threshold to mount expensive systemic inflammatory responses unless absolutely required...

Children infected with COVID-19 are at higher risk of developing diabetes.
This article published in June 2021 discussed two NIH-supported studies confirming SARS-CoV-2 can target and impair the body’s insulin-producing cells.
Type 1 diabetes occurs when beta cells in the pancreas don’t secrete enough insulin to allow the body to metabolize food optimally after a meal. As a result of this insulin insufficiency, blood glucose levels go up, the hallmark of diabetes.
