NZ Emergency Doctor Describes Crisis Caused By Lifting COVID Public Health Measures

scoop.co.nz/stories/HL2206…
The World Socialist Web Site recently spoke with Gary Payinda, an emergency doctor in Whangarei in Northland, one of the poorest areas of New Zealand, about the out-of-control spread of COVID-19 and the crisis in public hospitals.
In recent months, hospitals have been overwhelmed by COVID-19, influenza, and other respiratory illnesses.

Can we talk about medical professionals refusing to treat patients unless they expose themselves to COVID? As a self-regulated profession, how has medicine accepted this? How is this not being sanctioned as unethical? How is this already normalized? 1/x
As Professional Engineers (PEng), another self-regulated profession in Canada, we learned that our duty to the public is Paramount and that our right to self-regulation depends on that priority being maintained. 2/x
The PEng system in Canada was modeled after medicine and law. Medical doctors were held as the reference most trusted professional by the general public, a standard we were to aspire to. 3/x

So…staff wearing masks in the NHS now is just ‘strongly recommended’. Pray tell how the clinically vulnerable can safely access healthcare? Lots of very upset patients and I’m not surprised. Surely this is the least we can do? I know not all will agree but I’m disappointed.
In the same spirit, access to visitors in a and e is still restricted. Yet the staff don’t have masks on. Can someone tell me how this makes any sense? If masks are abandoned it’s not fair for patients to be in a and e on their own. Make your mind up @sajidjavid #MedTwitter
Who on earth is looking out for the clinically vulnerable? They have a right to access healthcare settings in as safe a manner as possible. This particular part of the pandemic (which is clearly not over, we have a new wave 🌊) will not go down well in history.

1/🧵 95% COVID Immunity⁉️

@JAMA_current data show that in Dec 2021, 95% of US Blood Donors had Immunity from infection or vaccination.🧐

Then why do we have record high rates of #Omicron?

Does that mean we should have herd immunity?

Not so fast…

bit.ly/3n2o5HD
2/ First, we make estimates using a combination of the COVID spike Ab & nucleocapsid Ab to determine what proportion of our population is protected at a given time.

ie, either previous infection (nucleocapsid) or vaccination (spike Ab) or combined Infection & vaccination.
3/ But if there was 95% immunity in 2021, how did #Omicron go bonkers infecting so many in 2022?

The infection rates reflect #Evolution. The I ntense contagiousness of this variant & enhanced immune escape mutations plus waning protection from previous vaccination & infection.

Let's try and pretend that Japan's high mask use has nothing to do with its success in limiting #covid19 deaths. Let's pretend this is because it's an island and that having the world's most populated city is irrelevant. Let's pretend they had full lockdowns (false). #Masks
Let's pretend it's because they are younger, so fewer die.
worlddata.info/average-age.php
But what about population density? Don't go there. Japan has the highest population density of any country with over 100 million people.

🧵 If the use of airborne #PPE & improving air quality in healthcare settings are to be ‘optional’ & ‘a matter of personal choice’, then we should give strong consideration to applying the same rule to the following #MedTwitter #MedEd #TeamGP 1/n

What I have learned since being unwell myself has destroyed any faith I had in my profession’s leaders. I used to think that deep down most senior clinicians acted because they had the best interests of the patient, their colleagues & society at heart. That illusion is gone. 1/n

@RobbieForChange lamented in a public lecture yesterday that US public health policy is constrained by what Americans are willing to do...
🤔 I would argue the chief constraint is @CDC inappropriately messaging safety with a pretty pastel map while we are in a surge 🧵
A recent poll showed that 74% of Americans would #MaskUp if we were in a surge.

ipsos.com/en-us/news-pol…
But @CDCgov hasn't declared a #Surge.

Instead of holding a press conference and sending alerts on social media about the impending threat of COVID in coming weeks, they "forecast" thousands of deaths.

COVID deaths are not rain. They are preventable.

It has been an honour to walk alongside my friend & colleague of 21 years @BinitaKane & her daughter Jasmin on their courageous & inspiring journey. #LongCovid in adults children is real & serious. There are treatments available- sadly only for a fortunate few #TeamClots 1/n
(Everything I say is with the express permission of @BinitaKane). Jasmin is not the only child with #LongCovid who has been shown to have microclots & hyperactivated platelets. They are a consistent finding in kids & adults who have travelled for treatment to Germany & SA 2/n
Globally 100m are affected by this vile illness. In the U.K. alone this figure is estimated to be 1.8m, with 2/3rds reporting an adverse impact on their daily activities. 1% of primary & 2.7% of secondary school children fulfilled the criteria for #LongCovidKids 3/n

1/🧵 What does it mean to be human?

How do I magnify human dignity?

I face this daily as we have dying ICU patients.

Recently 2 young men w #COVID, both progressive lung failure, chose not to be put on a ventilator.

Both young & fearful they wouldn’t survive if put on vent…
2/ I kept turning up HiFlow & Bipap. At one point each asked,

“What treatment is NEXT, Doc?”

Knowing I’d exhausted our solutions, I looked up again into each man’s eyes.

Honesty is the only way...

“Sir, your disease pushed beyond our tools. There’s no ‘next.’ No cure.”
3/ “What are you saying, Doc?

Then we went through his options again, which were narrow.

Talked with family, a holy time for all…still, no vent…

“Doc, are you saying I’m going to die? Wait, I want to go home.”

High levels of an #infection in #Massachusetts is causing both gastrointestinal (#diarrhoea, #vomiting) & respiratory (#cough, #sinus, #headache) symptoms WITHOUT typical fever or runny nose - School children & parents/guardians across #boston suburbs affected for last 3 weeks.
Note the CDC data suggests very little SARS-CoV-2 (Omicron) or influenza (flu) at this time in Massachusetts or Connecticut or New Hampshire. So diagnostic tests being used are not catching this circulating pathogen, or a seasonal coronavirus/norovirus/adenovirus is the culprit

2) The study included 61 school districts (kindergarten through grade 12) that provided data from July 26, 2021, through Dec. 13, 2021, a period encompassing the Delta surge and preceding the Omicron surge.
3) In total, there were 40,601 primary infections acquired in the community (36,032 among students, 4,569 among staff) and 3,085 secondary infections acquired in school (2,844 among students, 241 among staff).

I want to start out by saying, you folks are in a tough position, being PUBLIC SERVANTS of WE the People. I would like to believe that you truly have the best interest of our children and the community in mind, but you are factually acting outside of your delegated authority...
by practicing medicine, imposing unlawful medical interventions, and by simulating legal process.
That being said, our forefathers put together one of the most powerful and amazing documents in our history. The #constitution, which was drafted to protect individual rights!
They did this in an effort to make sure we didn’t end up under #tyranny again, and yet, here we are with individuals and Agencies operating outside of both their delegated authority and outside of the law. I’d bet my LIFE (emphasis added), if any one of them were alive today...

#BA2variant update THREAD

I told y'all it was going to be a problem back in December when it was discovered and have been warning every week what is coming as the data has become more available. It is 33% more contagious & antibodies from previous infections are irrelevant.
Reinfection is real with this variant.

Part 1

#Covid19 #BA2 #BA2variant #Omicron

Part 2

What really scares me are the variants that are going to follow and the toll that BA.2 is going to take on our already crushed Healthcare system.

1/🧵🎥 Politicians & Media are sending messages to take down #COVID protections.

In US it’s premature. Why?

📍>50% adults are NOT protected w #Booster
📍>75% of kids 5-11 are NOT fully #Vaccinated
📍Feb 8th >3,000 died
📍>17,000 dead in past week
📍>60,000 💀 January

Thoughts?
2/ Last week I had moments where I thought, “It really is sad that society has to keep masking up & those poor kids in school haven’t seen people smile in so long.”

THEN…I thought about how dangerous it is for #CovidKids who get #longCovid to have to live that way.

And…
3/ People continue to contract #Covid from children who skate through #Omicron initially while adults spend 3 weeks in ICU & die.

Denmark 🇩🇰 Israel 🇮🇱 South Africa 🇿🇦 will be safer faster.

US 🇺🇸 will lag behind by YEARS.

Messaging about an end to the pandemic is premature!
