So let's unpack this.

We hear a lot about "#dashboards"; we were even promised sight of one, back in March* - though we've not seen it yet.

But what are they, in reality?

Well, for starters, we know quite a bit about @NHSEngland's 'dashboard(s)': back in March, @MattHancock gave it powers to DEMAND confidential patient information from care providers*, for (broadly defined) "#COVID19 purposes"...

Under the #COPInotices:

...and we know @NHSEngland's hoovering up *at least* 70 different datasets into its #DataStore (the list at data.england.nhs.uk/covid-19/ is no longer comprehensive due to the controversial contents of some of the datasets).

As its "#PrivacyNotice" points out,

Adding to this. I see some resurfacing of the voices who felt she should have retired prior to 2016

It was on the rest of us to do our part..

..not for us to tell a woman (who worked 3-5x harder to get to her position than her peers on the same Bench) to leave early
She had a point.

“When that suggestion is made, I ask the question: Who do you think that the President could nominate that could get through the Republican Senate? ‘Who you would prefer on the court [rather] than me?”

#RBG was describing #gametheory here because she considered her move as well as all the other players’ moves and what the Nash equilibrium would be in the “prisoner’s dilemma” would the move being suggested to her really result in the outcomes others wanted? Image

@bruce_y_lee succinctly says what I lay out in multi-tweet threads

This is it in a nutshell

Divide and conquer is as old as time. Ask anyone whose history includes being colonized

#Healthcare is 1/5th of the U.S. economy. You bet non-clinicians want a piece of that
If you are someone who touches patients, has ever touched patients, provides direct care, has any clinical background, I'll be totally frank, I respect your role. If your salary adds to the 30% administrative overhead of #healthare in America, then I have questions about #value.
This is a framework my coauthors and I offered on how to assess value and cut waste. Let's start with administrative burden. What exactly do you get paid to do in #healthcare? (we should be asking) What does your job do to deliver on the quadruple aim?

This is a truth few want to admit. Rural health or inner city health depend on the full range of clinician types. When was a CMO of a 14-center FQHC with $100m budget, we already had massive staff turnover & burnout - ~50% loss in 2 years with hard to fill staff openings.
True, don’t have too much respect for those who simply want to fast track to “medical” cosmetic procedures to compete with plastic surgeons on Botox injections

vs extending needed care in primary care fields

but it is a free country

every healthcare loophole gets exploited
Please spare me, tho, attempts at superiority of who has more/less heart/ethics/intellect by training

Lots of fraud by in healthcare by

...humans

#equalpayday today. Full time working women in Australia earn 14% less than full time working men.
There’s a lot we can do about the #genderpaygap. Here’s a start. 1. Better value the work done in highly feminised occupations. Workers in the health and care professions perform enormously skilled and economically important work. They are underpaid. #paythemmore 1/
2. Organisation leaders should undertake a gender pay #audit to diagnose #genderpaygap drivers. @wgea has great resources for this. Because this is such a pervasive problem, there will be issues, be open to that and #actonthedata 2/

What if the money 💰💰💰 spent by #academia & #hospitals on hiring people to spy on #SoMe, to police & shame nurses & doctors (even getting some fired for speaking up on #PPE shortage), this policing disproportionately affecting BIPOC, instead spent on hiring BIPOC & on pay gap
This is pay gap I am talking about. What kind of “#professionalism” accepts that Black women must work 7 extra months for same pay?

Professionalism on “you wore..” is used in sinister ways against Black women in particular

#blackintheivory #MedTwitter

Puritanism in #medicine, being taught to med students who are given middle author status on a publication conceived and supported by senior faculty and junior editors, is part of this harm. Many Black girls are criminalized & punished for natural hair.

From @AmerAcadPeds - staying safe while Black or minority

To go a step further, we have police in #schools and even #school is unsafe for black #children and children of color

linkedin.com/posts/usnehal_…
What is the image and coloring of “an angel” or “a cherub”?

Not a Black child. These are powerful images that inform how people view and then treat children

Who is “innocent”?

Ask any advertiser. Images are powerful and change behavior ImageImageImageImage
There is suspension and punishment of Black girls and boys as early as age 3 called “preschool to prison pipeline”

What happens when the place you are supposed to go to grow, develop, expand, play, take risks

...is a place you are seen (and told you are) bad? Criminalized? ImageImageImageImage

This is the single best advice for any doctor at any stage: listen to nurses. In particular, listen to Black nurses

Also, protect nurses. In healthcare the terminology is “writing orders” - if that is not power imbalance, I don’t know what is

#MedTwitter
#NurseTwitter
#MedEd
One thing I have noticed on social media is: even at pre-med stage, the condescension & disrespect towards nurses sometimes is already there

Much of recent #MedTwitter drama I engaged in was because, though I knew it would be wise to “look away” could not

not if vs Black nurses
Across the board, regardless of race, when nurses, or anyone, is bullied within in clinical practice settings - setting that already have high rates of error

unsafe
on many levels

it is anti-#safety and #quality for patients & staff

Thread: For a good couple of years now, I have heard @makemytrip stories through #youtube videos, have admired the progress, and got inspired. Today had a privilege to listen to #MakeMyTrip Founder @DeepKalra at @headspin_io #Automation #Hackathon wrap. (1/N)
Trying to list down a few things which I could capture from all the wisdom which was flowing and ones which I will probably remember for a long time.

1. People (#customer) have to come back for a reason. Make sure to give them that. (2/N)
2. Sales were saying go back to the #Corporate job but the heart was saying "Too Early to Give up".

3. Customer Retention is the most important thing.

4. On #UX: I (Customer) am going to give you 5 minutes of my life to you. You have that much time to grab the eyeballs. (3/N)

Let’s talk #safetyculture

@TheIHI

It’s a framework for a harm reduction towards all: patients & families as well as all staff

Important:
growth mindset
instead of
fixed mindset

to have needed uncomfortable conversations

#MedTwitter #AcademicTwitter #MedEd #NurseTwitter
This framework has been applied to reducing harm, mostly through medication error (though intended to achieve population health)

Then was expanded to patient experience

Also, cost of care

More recently we have added clinician wellbeing

=quadruple aim

ihi.org/communities/bl…
What has not been clearly stated in any of these #safety framework is disparities, structural inequity, or racism

The acute care side of the hospital - money maker - thinks of this as type of safety & #quality

but not #populationhealth

#Implicitbias elicits 🙄😡😴 response

Yes.. and more than offended, I would reframe to respond with: your obvious bias reveals you are not a person who can be trusted to apply standards fairly for good outcomes.

Bias = unsafe.

Not about anyone’s emotional or intellectual reaction.

About #safety & #quality.
Discussed w/ that frame w/ @becker_sbecker

Look, I’ve got “grit” & resilience & can manage my emotions just fine

But are *you* safe or able to provide quality results? Using my former #Medicaid medical director lens, low quality = low value, waste, harm

go.beckershospitalreview.com/scott-becker-i…
“Safe spaces” emotionally, yes - but safe for whom?

Is safe space code for protecting your fragility when hard, complex problems are identified & you must confront some unpleasant realities?

Hard problems are... hard
Hard things are uncomfortable

🇵🇰 parents - when my parents born colonial rule, pre-Pakistan
To thoroughly annoy my Twitter bestie, @DrvanTilburg

Am an ENFP - a cross between Snoopy, Robin Willians, Ellen Degeneres, Dr. Seuss, Charles Dickens, Upton Sinclair

I border on ENTP - love new info and love to challenge in order to break through barriers or limitations
My great-grandfather was given title "Khan Bahadur" (brave). As a judge, a brown man under Colonial rule, he threw the book at a British attorney. "Learn the law" if in his courtroom. Hence my stiff moral backbone

📰🔊This is our analysis of the #COVID19 #testing strategy in #Tunisia🇹🇳, given the potential technical and contextual limitations, and future challenges.

Happy to contribute to @OpenNiqash with @Jaber_Bel, @drziedbouslama & @droussamazekri!
Real-time RT-PCR (@WHO gold standard) is complex and requires technical equipment and skilled human resources. Deploying a military mobile lab for temporary missions in the south is a good step, but still, testing capacity remains limited: only 24,406 tests performed as of 02/05.
Antigen and antibody rapid tests provide a less costly and faster delivery of results compared to RT-PCR. However, before these tests can be used, it must be acknowledged that they serve different roles and thus need to be used in different scenarios.
