Jason definitely struck a nerve for many of the #PharmEd & #TwitteRx folks. This is something we’ve discussed quite a bit in Maryland, but it sounds like a larger @ASHPOfficial / @ACCP task force is needed. A few thoughts… (🧵1/10)
1st: I am very proud to be a #pharmacist & love mentoring pharmacy students & residents on research projects every year…but…the large majority of PharmD grads *do not* have the knowledge/skills to *lead* a research project. We have to acknowledge that. (2/10)
2nd: (Trigger Warning ⚠️) A decent number of faculty members & clinicians are not strong research mentors. Even a faculty member with a PhD can struggle mentoring residents, because mentoring is a skill that needs development - but we just expect it as a given. (3/10)

Because he’s an ENTIRE PERSON.

So every minute with him is a gift. What did I learn?

Burnout is an epidemic.

It ruins careers, relationships & mental health.

A study by Deloitte reported a staggering 77% of workers have experienced it at their current job.

But it doesn't have to be this way, says @NathalieRach.

Her Top 4 strategies for avoiding #burnout.
1/ Seek inspiration

Quite literally everything is more difficult if we don't believe in WHY we are doing it.

From careers & fitness, to academic pursuits, spending the time upfront to identify our purpose & motivation can provide us the tailwind we need when things get tough.
2/ Invest in connections

Research shows that deep, personal relationships can impact our physical & mental well-being as much as fitness, sleep or nutrition.

We should treat them this way. Be honest, be vulnerable & seek a few deep, quality connections over many shallow ones.

1/🧵 Seeing Differently in #COVID ICU Care 👁

The world tells me “stuff” & “freedom ” bring happiness, yet neither suffice or satisfy.

If I reject chances to serve my suffering patients, I am left bereft.

Merciful care brings healing & provides joy all around.

What is #Mercy?
2/ My working definition of mercy is my willingness to dive into the chaos of another person’s life & provide lifting & healing.

Often as an ICU physician I do the former but not the latter.

That is, I dive into their #chaos without admitting I don’t know what’s right for them.
3/…but without performing actions that lead to lifting and healing of my patient’s spirit & mind, what I’m doing is dispensing false mercy.

eg, If in #COVID, I make the lungs 🫁 my whole goal, sedate & depersonalize someone for too long, I beget new diseases.

Early on as an ICU Doc, I carried a lot of shame because I realized that even though I sufficed as a technician and possessed the right knowledge, I was NOT doing a good job as a “person” with “people.”

2/ Which is to say, I was only “good” doctor on paper. NOT in real life.

I kept myself at too much of a distance, and I thought my job was to do procedures well and provide the right medicines.

Now I know that was not enough, not by a LONG SHOT.

Vicariant: qui se substitue à. On appellera le PtV cet impact psychotraumatique du vécu d'autrui, de nos patients, et la "contamination" traumatique de ceux qui les prennent en charge, suite à l'exposition aux récits et images impactantes.2/18
De façon répétée. Tauma par procuration, donc. A différencier du #Burnout et de #épuisementprofessionnel. Le PtV n'étant pas une surcharge de travail (quantitative) mais l'impact psychotraumatique spécifique de ce travail (phénoméno). Bien que l'épuisement pro soit un FDR. 3/18

I’m a #triplevaxxed surgeon, now sick & isolating with #COVID19. I’m married to a #triplevaxxed rheumatologist MD & mom to 2 kids in Moderna 💉trial, all negative but #quarantining since 3 y/o is blinded to vax status. Wanna know how Hubby’s work is treating him? (A 🧵)

He works for a large private health company, so Families First Coronavirus Response Act /FFCRA doesn’t apply. He is staying home with the exposed kids (kid 2 is 3 yo & still blinded in the trial). His options?

2️⃣Take unpaid leave…
He will likely test + in the next few days given the household transmission of #Omicron, and then will STILL either take unpaid leave or use our much-needed vacation time.…

Observations on academia as we move into another term impacted by the pandemic
- everyone is exhausted, lots are fearful about the coming months
- unis are still not attending to student/staff need
- wellbeing isn't understood in terms of work/teaching practices
- accommodations students and staff are entitled to have still not been met
- the impact of Long Covid is barely recognised
- staff and student concerns about Covid-19 safety are being ignored
- many conflicting messages re: health and safety

- because of the legacy of bad academic practices prior to the pandemic and poor management from unis since, trust is very low
- therefore where support is offered students/staff are wary
- where excellent support is available people are too tired to take it

Truthfully, I’m feeling more bitter than sweet entering this new year, especially when I compare it to how much hope I had entering 2021. I rounded out my last two weeks of the year on the general medicine wards, taking care of both COVID and non-COVID patients. My experiences:
-Escalating care for COVID patients but ALSO:
-Taking care of an unstable post arrest patient for 1.5 hours on the floor because there were no immediate ICU beds
-Self doing Q1H checks on patients not stable enough for the floor because there were no immediate ICU/SDU beds
-Unable to discharge patients swiftly with anything beyond a “home with no services” dispo because everything from STR to psych to delivery of equipment is back logged (seeing a theme here?)
-Extreme medication shortages, including blood. Only getting a HALF unit at a time…

Zwei Jahre hab ich gekämpft, meinen Job als Oberarzt der Unfallchirurgie auf 60% Teilzeit zu reduzieren
2/6 um mich um Kinder, Haushalt und Soziales kümmern zu können. Arbeitgeber wollte nicht. Hab geklagt, Arbeitsgericht und Landesarbeitsgericht gaben mir Recht. Ich habe ein Konzept ausgearbeitet wie in der Unfallchirurgie Teilzeit gearbeitet werden kann.
3/6 Die Hälfte unserer Abteilung arbeitet 80%, nachdem ich damit vor 5 Jahren der Erste war. Es klappt super. Dadurch haben wir drei neue Stellen generiert. Weniger Wochenendarbeit für alle. Mein Chef, alter weißer Man, hasst mich dafür.

There is someone in my life who is a pilot who disagrees with me on Covid approach. This person is now working during the current airline crisis. I wrote to them on using the airline crisis as an opportunity to understand the healthcare crisis. Thought I'd share here:

What is happening in the airline industry is happening in healthcare, too, only, unfortunately (for reasons that go back hundreds of years and are complex), physicians don't have unions.

Imagine if you, as a pilot, were told you had to fly a plane in unsafe conditions.

You'd refuse and go to your union, if needed.

Well, what if, (go with the metaphor here), if you didn't fly that plane, all the passengers would die?

(Imagine they have to get somewhere for lifesaving treatment, for example)
