I am currently at home in isolation with (mild) COVID. I’ve been thinking about why kids & teen COVID vaccines AND BOOSTERS are really important, right now. Here is my long 🧵:
Pediatricians deeply understand risk/benefit of vaccines. As a peds ID doc, I see kids who fall through the "prevention cracks" including immeasurable suffering from severe tetanus, measles pneumonitis & SSPE, chickenpox encephalitis, pertussis, meningitis, and now COVID.
Situations of telling a gasping teen to “lay prone” are vaccine-preventable. This pt was also alone, because her parents were at home, also sick w COVID pneumonia, and too ill to accompany her on hospital transfer. They were all previously healthy and unvaccinated.

#MedTwitter Hi again from your neighborhood cardiologist - This article from the UK is getting quite a lot of interest because it has worked harder in determining the incidence of COVID vaccine myocarditis after boosters. Let's think through this one. medrxiv.org/content/10.110…
#Vaccineswork #COVID19 #COVID19France #COVID19Vic #tweetiatrician The structure of their study is essentially that they looked at 42 million Brits >13 years of age and checked, based upon national medical records, whether or not they had COVID disease myocarditis, Vax myo, and...

Should #children be vaccinated against #COVID19?

Our review highlights that the balance of risks and benefits of COVID-19 vaccination in children is more complex than in adults as the relative harms from vaccination and disease are less well established in this age group.

#LongCOVID

We reviewed 14 studies of persisting symptoms following #COVID19 in #children & #adolescents

Most had major limitations and should be interpreted with caution.

True risk is likely to be nearer 1 in 100 than often quoted 1 in 7.

The 14 studies involved 19,426 children & adolescents.

Common persistent symptoms reported 4-12 weeks after acute #COVID19 were:

•headache
•fatigue
•sleep disturbance
•concentration difficulties
•abdominal pain.

Symptoms rarely lasted more than 12 weeks.

A brief summary of science literacy in the US, with extensive citation credits to @ENirenberg , @ShawnOtto , and @neiltyson : Why its so important, why the US still has issues with this, and what you can do. #medtwitter #Immunology #VaccinesWork #tweetiatrician #pediatria
The explosion of misinformation, disinformation, and anti-medical establishment sentiment goes way back, even back to the invention of the concept of vaccination. One source reports this maybe goes back to at least the time of US President John Adams smithsonianmag.com/history/age-ol…
If you ever wanted to make a scientist upset, tell him/ her that science has political power. Why? Because if you were a child, poor, and could get a scholarship to school, your science experiment was on a level playing field with the rich expat child who conducted an experiment.

I keep seeing hot takes on how covid affects kids + if it’s dangerous for them.

But amazingly there’s still a striking shortage of pediatrician voices given the opportunity to provide our nuanced and expert opinion.

What would we say if you asked us?

A thread 🧵
Pediatricians have a lot to say about the covid in kids, how to balance the risks and benefits of exposure, and how to make personal + public health decisions that prioritize the needs of children.

What would we say if asked how dangerous covid is for kids?
We’d ditch glib mortality statistics that dress up data as science + paint a more complicated picture.

We’d reassure you that children continue to mostly do well with acute covid. We’d then explain that deaths + hospitalization from acute covid are just part of the equation.

Team it's time to address the Hoeg et al preprint on post-COVID vaccine myocarditis. How am I qualified to speak about myocarditis? I am board certified in paediatric cardiology and currently have patients recovering from this condition. TLDR: They are doing quite well. #MRI
What is the premise of that article? directly scanning VAERS data will result in capturing more cases of myocarditis, and therefore the "conclusion" that COVID vaccine generates a higher risk of myocarditis than COVID disease. But - cardiologists have diagnostic criteria. #heart
All that bleeds troponin is **not** myocarditis. I cite my professional society, the American College of Cardiology: #myocarditis #COVID19 #vaccination #VaccinesWork Image

To reassure parents, a reminder of some key facts about #COVID in #children:

#SARSCoV2 infection remains asymptomatic or mild in the vast majority of children, even with the #DeltaVariant. Hospitalisation and deaths from COVID are still rare in this age group.

1/n 🧵
However, the greater transmissibility of the Delta variant means there is more COVID in *all* age groups, including children, so we will see more paediatric cases.
As a large number of older people are now vaccinated, the increase is blunted in the elderly so, compared with earlier in the pandemic, a higher *proportion* of current cases will be in children: this can give the mistaken impression that the Delta variant is targeting children.

We are officially back to getting crushed by #COVID, and for #pediatricians this wave feels *worse* in many ways. And as a pediatric hospitalist who has been a front-line COVID doctor for the entire pandemic, I’m going to share what I’ve seen the last 8 days on service. 1/
After many months of zero or few pediatric COVID cases, we are seeing infants, children, and teens with COVID pouring back into the hospital, more and more each day. These patients ranged in age from 2 weeks old to 17 years.
COVID admissions in our medical center are up 500% and currently 80% of cases in our hospital are due to the highly contagious Delta variant. I worry that the Delta variant, in addition to being more contagious, *may* also be more virulent in children.

Can we talk about all the “kids don’t need masks” hot takes from “experts” that keep cropping up?

I’m exhausted and frustrated and just need to lay some thoughts out there

A 🧵

#pandemic #parenting #tweetiatrician #medtwitter #covid #vaccines #masks
I’ve been bombarded with DMs, comments, articles arguing for a back to normal for kids that ditches all masks for kids + insists covid isn’t bad for them.

The main tactic is the same old using stats + missing the big picture to ultimately argue covid isn’t dangerous for kids
The reality of covid risk to kids is much messier. It involves understanding the complexities of mortality rates, how all children deserve to live a life free of vaccine-preventable disease, how the known + unknown long term effects scare pediatricians more than acute disease

It’s the first week of residency for many incoming residents this week. I remember my first day @usask so vividly: I was so stressed out that I developed tinnitus and asked my senior to examine my ears because I thought I was suddenly becoming deaf. Everyone: take deep breaths.
I have one advice: you are NEVER alone. Don’t ever think that you need to suffer in silence. If you don’t know something, it’s normal! You’re not supposed to know anything: that’s why you’re where you are. No one expects PGY1s to be experts in their fields.
Your co-residents will become your family. You will look after each other. That is so precious and will last your entire career. But also remember that your staff is here for you: you need a shoulder to cry on? You need a hug? You need a warm supper? We’re all over this. Call us.

This is why I do long threads on racism in child & education spaces (child education up to #MedEd) and on the role of women to “protect” only some kids, not others. There is focus on police without looking at the full picture of sources of harm to children of color.
It is very very easy to focus on the Boston police Union leader

but who is it that..

helps him get access to children?
stays silent?
silences abused children?
promotes him?
seeks to please him (be promoted by him)?
sees him as a “good leader”?
softens his image?
protects him?
The role of men & women who police/silence those who would speak up... who prevent identifying the abuser

=extremely important

it is all on one spectrum: if you experience being denied food by a teacher as a 3-year old, will you speak up on abuse...

vs Stockholm Syndrome?
