Also, at midcareer female success is threatening to men our age AND to younger men who rely on patriarchal patronage systems. Women, overpoliced by the system, then, yes, are tougher bosses AND we *do* tend to adhere more closely to ethics.

I do do all:
On #MedTwitter there is a pattern of “callouts”, yes vs all genders & ages, but also pattern of misogyny and misogynoir. It happens towards nurses & doctors alike. One of the reasons I connected with @ShrimpBrokkoli is because I saw a yt male resident ridiculing her online.
I saw that behavior being taught by NON-clinical faculty & that REALLY crossed a line.
If you are a woman + nurse + frontline + Black/WOC, you already live with so much violence towards you. Added online made up drama from a trainee vs nurse?
No, no, & no.

This is happening in U.S. & U.K. Once I got to the management & C-suite, especially in #NYC, I was unwilling to send #HCW into unsafe spaces to be injured or die.

@drbreenheroes this is the crux of the issue. “#Mentalhealth” is really #moralinjury from #ethics violated.
The frontline is disproportionately POC, women. Those making guidelines or in C-suite are able to protect themselves. Leadership manipulates data/definitions, then shames, gaslights. Non-clinicians hired to override #HCW with “opinion.” Those at frontline who speak up: fired.
Who is among these flags (& whose loved ones)? It is not those pushing out pre-prints or on TV nor those safely behind a screen from a second home. It is those at the frontline, working without support, protection, or even without pay (some hospitals asking docs to volunteer).

This is what targeted racial harassment looks like: nitpicking things like “smiled” or “wore clothes like a celebrity, as a celebrity” or “bought cookware.” It is part of the “gold digger” narrative.
This is common in #healthcare#professionalism” where there is hazing & bullying by racist &/or classist made up non-rules lacking standards…yet people are called “breaking the rules.”

It’s exactly how bullies operate: double standards, mislabel/smear, exclude, marginalize.
In the U.K., “BAME” includes the formerly colonized, thus making race constructs or power different from U.S. as latter had race-based chattel slavery.

Thing is, those formerly colonized have many who have internalized self hate & racism.

This makes me so 🤢

The idea that women are automatically “safe” is exactly what creates this “opportunity” for some women to exploit children & monetize their “optics” of “trustworthy” on behalf of predators.

Gender does not predict ethics, integrity, safety.
Why does #GhislaineMaxwellTrial matter to #womeninSTEM?
⬇️federal funding
➡️private donors (Epstein)

“Socialites” open doors to elite spaces, including #academia #science #STEM
Women=“optics” & “reputation management” for these wealthy predator men

theverge.com/2019/9/19/2087…
These same connected women, often daughters of powerful businessmen

are on boards of #WomeninSTEM “empowerment” organizations or on #MeToo (TimesUp scandal)

the very women who are predators of young women are given access to women’s “safe spaces”? (!!!)

#antivaxx efforts now even use #deepfake

to target #refugee, Black, religious minority communities

like measles outbreak in Minnesota in 2019: Wakefield was flown in TO spread #misinformation & #disinformation

Now: online fake accounts/fake identities vs #VaccinesWork
What has #healthcare been doing meanwhile?

#Medbikini: Teach (male Asian) med students to make fake accounts to cyberstalk trainees’ personal Facebook for: bikini
or “being political” or “religious” (#BLM)

➡️“model minority” pipeline for “shadow faculty” vs Blck @ayshakhoury

He spent his life as a first responder when others needed him. When he had a heart attack on the job, system overwhelm meant no resources available for him.

This is betrayal.

We’ve been asking, pleading, negotiating, screaming for this not to happen.
Those #HCW who spoke up early on #safety? Fired.

bloomberg.com/news/articles/…
Maybe listen to the ones who care enough to speak up on REAL #safety issues rather then fire them.

The current #HCW correct dismissals over refusal of #VaccineMandate claim administrators are “bullies” because that HAS been true in past.

A few thoughts on #Omicron and our healthcare system:

1️⃣ Our system is ALREADY overwhelmed- hundreds of thousands of people have been denied care and are waiting for critical procedures- the existing backlog will take YEARS to clear.
2️⃣ Triage IS occurring NOW - people are dying who have been unable to access care because the acute care systems is overwhelmed with #Covid. Thousands, yes thousands, of 🇨🇦’s have lost their lives for this reason.
3️⃣ Cancer screening has dropped by 25-35%- these people will show up eventually - they will be much sicker and will need more resources to treat them and their outcomes will be worse than they could have been.

Those of us with immigrant backgrounds have NO idea what debt we owe these women for risking their lives so some of us can shop at Whole Foods with other suburban moms, then drive a Lexus home to ride on a Peloton, & think only our own hard work created this access to success.
Those of us who are Muslim have a partial experience of what it is like to be made the face of threat and danger and vilified.

…But “the West “still wants to “save” Muslim women… make us a token or mascot of saviorism & evidence of being a shining city on the hill.
In comparison, the black woman doesn’t have that opportunity to be paraded around as the most recent woman “saved” & celebrated - the tropes are universally harmful & exploitative/pejorative vs Black women in society/media + centuries of economic barriers/being exploited.

"defendants…willfully destructed the evidence by deleting the webpages and social media accounts," Hanna wrote. "Plaintiffs cannot continue to be blindsided by the defendants by having to search for what evidence is being destroyed or altered”

This is a really fascinating case as a lot of what I see happening in #MedTwitter (Twitter presence of the legal minefield that is #healthcare #MedEd, etc):

faculty, consultants, #hcw deleting content & accounts

@HarvardHealth tells us how #COVID19 is spread. “Aerosols are emitted by a person infected with coronavirus when they talk, breathe, cough, or sneeze. Another person can breathe in these aerosols and become infected with the virus.” #ph260720

Do you know what an aerosol is? Choose the definition below:
1. A substance that is stored in a can
2. Tiny particles or droplets suspended in air
3. A chemical

My 11MO daughter needs surgery and an MRI to help our amazing #HCWs establish a diagnosis for her developmental delays. Both procedures and resultant time-sensitive treatments will be delayed as #Sask spirals deeper into this #COVID19 nightmare. 3/7

I lost my third #covid patient in less than a week, while working part time, in a rural area hospital that serves around 35k. Last I saw we have a #vaccination rate of 30%. I'm sitting here feeling numb, furious, exhausted, and... ashamed.
Ashamed because I realize some #HCWs have been at this since March 2020, losing multiple patients each day, and I'm wondering if I want to continue a path in #healthcare. I see headlines about 6 ICU nurses walking out in one day and can't blame them at all.
A close family friend is ending her prestigious career as an intensive care pharmacist in a world renown hospital to save her mental health. I've barely been in this 6 months and I'm so tired of knowing and seeing people die who could have avoided this.

I work in a small, rural hospital. Stayed extra after my shift yesterday to shuffle beds, grab call lights, and answer phones while the ER dealt with 3 traumas and a full house after dozens of COVIDs all day. At one point the ER was bringing beds with people to clear bays..
in anticipation of the 3 rigs bringing people in. I never got the full situation, but when I walked out over an hour after my shift ended there were still several law enforcement vehicles there. At one point, the ER had asked my unit to prep a room for a body just for family to..
say goodbye. Meanwhile, that same ER staff had had dozens of COVID patients, way too young, they were trying to admit but I kept telling them we don't have beds or staff (we don't). I could hear the breathlessness and exhaustion in the ER staff when they called to say, "we'll...

#MedTwitter influencers show hypocrisy (advice vs actions on masks). Agree:

“unpleasant comments are ‘the nature of social media’ and holding public figures accountable is critical when they are setting an example for their ‘millions of followers.’”

adds to public perception:

In light of the Boston police Union leader child abuse, this is very plausible, even probable. I have 2x, on behalf of someone else, reported, via Title IX - the woman or gay man was not feeling safe enuf to report. Yes, there is blowback when one reports. But we can’t be silent.
I am a mandated reporter as a #pediatrician. I *must* report for my patients or face legal consequences. Am same for staff, students, trainees. But in #EmergencyMedicine who do I call if a child may be unsafe at home? Sometimes, cops. Are cops safe?

boston.com/news/local-new…
I have seen #mentalhealth and entire life paths destroyed by abusers preying on children like: This happened to too many people I know. I have seen institutions intimidate, mislabel, bury stories of the victims. (Some predators wait until 18th birthday.)

Until we are all safe, no one is safe. Covid is a global problem ⁦@RobinMcKietheguardian.com/world/2021/jan…
It is reasonable & understandable for countries to roll out vaccines to vulnerable people & #HCW within country. But at the same time ensure that vaccines are made available urgently & equitably globally through ACT-A, @WHO, @GAVI & @CEPIvaccines
That needs intense planning, manufacturing at scale, trust, communication, logistics, people & teams, regulation, approvals, cold chains. That does not happen overnight. Urgent planning now for when vaccines are available is crucial.

Just because I am perpetually wading into controversy...as a BIPOC but not a Black woman... here I go again.

#BlackExcellence in #healthcare is VERY needed. Thanks for tweeting this @mdawriter

That includes #blackexcellence as physicians AND every other type of #HCW
This is super complicated. Traditionally, #healthcare has been physician-led "therefore" the ills of healthcare also fell squarely on the heads/shoulders (malpractice insurance) of physicians. That said, now most physicians are employed. Then the "therefore" of #accountability?
Workforce and representation very much are important for the 1:1 care. We know this to be true, for instance:

hbr.org/2018/08/resear…

But it is a long pipeline to get a practicing, licensed physician and requires a lot of debt/resources

This isn’t decision any government would want to make. The sooner we act, sooner we start to recover. A very difficult few weeks now & no one can underestimate toll on people. But consequences of sticking with the current insufficient restrictions would have been much worse.
"I’d like nothing more than to have a normal family Christmas. If we can steel ourselves now for a few weeks of greater restrictions, there’s a chance we could ease up a little between Xmas-NY. But if we’d let it continue to multiply, we’d be in a terrible situation in December.
