It seems the fastest way to get covid is to get vaccinated.

If you have been vaccinated and were subsequently infected with covid, how many days after your jab did you test positive?
@timspector have you done any analysis of this nature on breakthrough infections via ZOE?

I want to put this study on everyone's radar b/c I think it's a genuinely cool opportunity for people/patients to get high-quality data on their #SARS-C0V-2 #immune status over time👇 (I have no financial ties to the company).
2/ To be more clear about what the study involves, the @serimmune team will assess each subjects’ individual immune response to SARS-CoV-2 from both natural #infection and #vaccination for up to 5 years.
3/ If you register for the study, Serimmune will send you a sample collection kit that allows you to collect your blood at home. Then you simply mail the sample to their lab using the pre-paid envelope provided.

As #MedTwitter regulars, you may have already heard that the #SurvivingSepsis Campaign has released the updated 2021 version of the International #Guidelines for Management of #Sepsis and #SepticShock!

But have you gone thru the updates yet?

A #Tweetorial (1/x):
There are 93 #recommendations made in the new publication. I don’t have time to write (or read) that kind of #Tweetorial and neither do you. That’s what the publication is for ->… !

But let’s hit the ✨highlights✨ starring Table 1: (2/x)
Why we care (and we care so much!): Sepsis and septic shock affect millions of people globally each year, and kills somewhere between 1/6 and 1/3 of those diagnosed with the #syndrome. 1/3!!! 😳😢 (3/x)

3rd Dose Of Vaccine:

🦠2nd dose still protects against #hospitalization (92% Moderna, 85% Pfizer) and #death (100%)

🦠3rd dose is to protect against #infection

▪️Jan 2021: 86% Moderna, 76% Pfizer

▪️Jul 2021: 76% Moderna, 42% Pfizer

🦠Moderna has more mRNA than Pfizer Image
3rd Dose:

🦠Antibody levels decline over time

🦠Higher levels of antibody are associated with higher levels of vaccine efficacy

🦠Higher levels of antibody may be required to protect against the delta variant

🦠A booster mRNA immunization increases antibody titers by >10-fold Image
3rd Dose:

🦠Vaccine effectiveness against #infection (symptomatic & asymptomatic) is decreasing over time

🦠Vaccine effectiveness against #severe disease, #hospitalization, #death remains relatively high

🦠Antibodies going down, delta going up, add booster #before deaths go up Image

It’s well known that our society is infected by concepts which make us unhappy, damage our communities and endanger the biological world. These concepts infect us, we become contagious, then we infect those around us.

We know the sort of concepts they are. They look like this:
–You need more
–Take, don’t give
–Winner takes all
–Greed is good
–Appearances matter
–Those in power deserve their power
–Those with wealth deserve their wealth
–It’s your fault you’re poor

Some of these concepts simply misinterpret Darwinism. Some are demographic falsehoods. Some are empirically false. All lead us to a dangerous outcome for our species.
I consider them viruses–contagious & malign concepts which are part of a mind plague that’s harming us all.

1/ Serious End-of-Life 🧵

The latest #infection survey from @ONS shows a dramatic increase in S, with increases also in W and E and a small fall in NI.

E 🔼from 0.19% to 0.22% (1 in 440)
W 🔼from 0.07% to 0.12% (1 in 830)
NI 🔽from 0.16% to 0.14% (1 in 720)
S 🔼🔼🔼from 0.17% to 0.46% (1 in 220)

1/ Image
Here's the data, along with the Confidence Intervals.
It's clear that whilst the CI's are much wider for W/S/NI, the S increase is still very significant.

W has bounced around recently but seems to be on an upward trend too, as is E. NI appears more stable.

2/ Image
Regionally, the NW and NE are still on the increase, whereas elsewhere it's a mixed picture. Albeit at lower levels the SW and East are rising too, others appear stable or even possibly falling.

In a meeting I watched today, microglia priming was mentioned in #LongCovid and #ME/CFS. It’s important to clarify what “microglia priming” means. Microglial priming does not mean that after a trigger has “cleared” microglia remain perpetually activated
2/ Instead, microglia priming goes like this 👉 When microglia or other glial cells detect #infection, injury, or inflammatory mediators, they enter a state of activation in which they change morphology and release their own neuroexcitatory inflammatory mediators
3/ Then, after activating, they retain a “primed” functional state which causes an even more robust response to *subsequent* infectious/immune/#inflammatory challenges. And as cells, microglia live long lives (they are not replaced as often as many other cell types)

Preprint reports elevated serum inflammatory cytokine profile in #LongCovid subjects. Worth noting that an ongoing immune response towards persistent viral reservoirs of #SARS-CoV-2 and/or antigen could explain the findings:…
2/ So the findings underscore the need to do studies that obtain tissue (via surgery or biopsy if possible) to search for #SARS-CoV-2/antigen in #LongCovid patients. Similar to what this team did (and found viral RNA/antigen in multiple tissue types!):…
3/ Studying both the LongCovid immune response AND possible #viral reservoirs is very important for LongCovid patients to get the best treatment. If you assume the #immune response alone is the problem, standard of care could become immunosuppressive drugs

A thread on #Mucormycosis :

One of the lethal complications of Covid is Mucormycosis, also known as ‘Black fungus’ #infection. This dangerous fungal infection affects the nose, eye and sometimes the brain. My colleagues in ENT and Ophthalmology told me that previously (1/n)

....the chances of developing mucormycosis as steroids tend to increase blood sugar levels. My colleagues told me that when sugar level crosses 200 mg/dl (Normal range: 70-140), there are higher chances of developing mucormycosis. (3/n)

1/ Kevin from Texas asks: I just received the first Pfizer vaccination dose today & am scheduled to donate double red blood cells in one week. Should I reschedule? Will donating blood impact the success of the #vaccination? Image
2/ According to the @RedCross, if you received the Pfizer, Moderna, J&J, Novavax, or AstraZeneca vaccine & you're feeling well, you can donate blood--with no wait period. There is also no reason to expect that donating blood will impact your body’s response to the vaccine.
3/ It is safe for the donation recipient because getting these vaccines cannot cause #COVID19 infection-in fact, there is no SARS-CoV-2 virus in these vaccines at all!

Fun fact: this is the same reason that getting #vaccinated cannot cause a positive COVID-19 #infection test.

v10 of our #LivingReview is live on @qeios

We have reviewed 345 studies. 52 of sufficient quality for #MetaAnalysis to explore the association of #Smoking and #COVID19 #infection, #hospitalisation, #severity and #mortality.
No dramatic changes from v9 posted in November.

Most studies from 🇺🇸, 🇨🇳 & 🇬🇧 but from 39 countries. 65% hospital based, 8% purely community. Median of 461 individuals per study.

Smoking remains poorly recorded, only 29% of studies categorise as current 🚬, former and never 🚭
6 studies report on alternative nicotine products.

Large amounts of missing data in many studies on 🚬 status introducing bias.

Huge reliance on #ElectronicHealthRecords and all the limitations that brings. See our recent #PrePrint if interested (…)
