Cool new study which found that a range of #pathogens (some capable of long-term persistence in tissue + #blood) upregulate expression of the cell surface protein CD47 in the host cells they infect as an #immune evasion strategy
2/ I fact, upregulation of CD47 is a very smart survival strategy by such pathogens, b/c it interferes w/ the host innate immune response that is normally supposed to identify, target and kill them!
3/ Specifically the team found that upregulation of CD47 by the pathogens under study slowed uptake of dead/#infected cells by the #immune system, including the downstream activity of #antigen presenting cells that are needed to recognize pathogens in the first place

Let’s do a quick one on #clinicaldecisionmaking and #neuroimaging. See bright spot on MRI T2 Brain just means increased water -could be any insult to the brain - #infection #inflammation #ischemia #neoplasm are the top 4 you gotta have in your brain. Pun intended. #MedTwitter
Then you think - active or inactive process? If it’s actively destroying the BBB, it’ll enhance on contrast enhancement. So get MRI with and without contrast. Why without? T2 shine-through - but that’s for another time. #MedStudentTwitter #neurology #neurologyresident
Now the important question here is what you do - remember that nice poem on #theroadnottaken by #robertfrost? 2 widely diverging choices - to immunosuppress or not to immunosuppress. #tobeornottobe #hamlet #MedTwitter #AcademicTwitter #MedStudentChat

Obstructions of the ureter can be caused by a number of factors and affect the physiological flow of urine from kidney to bladder. To overcome this issue, ureteral stents which have side-holes bypassing urine from ureter to stent and viceversa, are clinically implanted. 2/7 Image
However, stents are prone to complications leading to infection. We employed microfluidics to mimic flow dynamics in ureteral stents in different obstruction (obs) conditions: 1) unobstructed 2) obs at proximal side-hole 3) obs in between side-holes 4) obs next to side-hole. 3/7 Image

1/ THREAD 7: Viral Latency and a Hematogenous Route to CNS?

Is it even conceivable that #SARSCoV2 could establish latency in sensory ganglia (like trigeminal ganglia in HSV1) and reactivate to cause recurrent disease as respiratory droplets could enter the eyes, nose, and mouth?
2/ Specifically, talking about the peripheral nerves of the eyes, nose and mouth here. And thinking along the lines of why clinicians are seeing the loss of smell and altered taste in so many patients. Please remember, I am not a #virologist or an infectious disease specialist!
3/ HYPOSMIA DATA:
Both the American Academy of Otolaryngology - Head and Neck Surgery AAO-HNS and ENTUK have classified loss of smell and altered taste as significant symptoms in #COVID19. #Covid19UK

New WHO & @HRPresearch shows that maternal #sepsis is resulting in more illness and deaths than previously understood: sepsis occurs when the body’s response to infection causes injury to its own tissues & organs

👉bit.ly/3bzj4ie
To save lives from maternal #sepsis, health facilities need to invest in:
✅Clean water and sanitation
#Infection prevention and control measures
✅Training, equipment and supplies

Some have suggested that detection of #SARSCoV2 antibodies could be a basis for an “immunity passport” / “risk-free certificate” that will enable individuals to return to work. Currently, there is no evidence those who have recovered from #COVID19 are protected from re-infection.
Immunity to a pathogen through natural infection is a multi-step process that usually takes place over 1-2 weeks. The body responds to a viral infection immediately with a non-specific innate response: macrophages, neutrophils, dendritic cells slow the progress of the virus.
This non-specific response is followed by an adaptive response i.e. body makes #antibodies that specifically bind to the #virus. These #antibodies are proteins called #immunoglobulins. The body also makes T-cells that recognize & eliminate other cells infected with the virus.

Thank you @PlzSolveCFS for including me this video that helps people better understand the devastating and debilitating disease #ME/CFS: m.youtube.com/watch?v=LPUxt3… Image
2/ A growing body of evidence indicates thst #ME/CFS involves neuroinflammation, possibly connected to the activity of #viral pathogens capable of persisting in the central nervous system (CNS): frontiersin.org/articles/10.33…
3/ There have been dozens of #ME/CFS #outbreaks over the years, many connected to #enteroviruses, coxsackie #viruses and even respiratory viruses (w/ enterovirus found in the brain of patients via autopsy in some cases) Image

When I started working on #scabies outbreaks in #carehomes some of my colleagues thought it was a weird, marginal interest. Now with #COVID19 it's changing. Everyone's realising that infection is a big deal in these #EndOfLife settings. Image
#Carehomes for older people are hard to study. Residents often have #dementia, staff are run off their feet, #consent to research is difficult - brilliant colleagues including @MedVetAcarology sorted this academic.oup.com/phe/article-ab…
Life expectancy on entry to a #carehome for older people is typically 8 months - these settings are largely #LastHomes. Everyone needs a plan for what will and won't happen for the common things likely to make them ill or deteriorate. #CPR probably not part of it

This study found that Epstein Barr #Virus (EBV) IgM antibody seropositive #COVID-19 patients had a 3.09-fold increased risk of fever compared to EBV seronegative patients. Inflammatory markers like C-reactive protein were also higher in the EBV+ patients: researchsquare.com/article/rs-215…
2/ The study is a small, but good example of the how already-acquired persistent #pathogens can influence how a person responds to a new #infection: the existing pathogen(s) might cause additional symptoms or influence how the #immune system can respond to the new infection
3/ For example this study found that persistent Cytomegalovirus (CMV) infection caused ~10% of all T cells in CMV+ individuals to be directed against that virus. Could harboring CMV then change how the adaptive immune system responds to COVID-19?: ncbi.nlm.nih.gov/pmc/articles/P…

There is no evidence that the Bacille Calmette-Guérin (BCG) #vaccine protects people against #infection with #SARSCoV2 #coronavirus causing #COVID19. But, 2 #clinicaltrials addressing this question are underway. WHO does not recommend BCG vaccine for the prevention of COVID-19.
There is EXPERIMENTAL evidence from animal & human studies that #BCG #vaccine has non-specific effects on the immune system. These effects have not been well characterized; clinical relevance is unknown. WHO updated its evidence review of major scientific databases on this topic
The review yielded 3 pre-prints (before peer-review), where authors compared incidence of #COVID19 cases in countries where the #BCGvaccine is used vs. countries where it is not used & observed countries that routinely used BCG in neonates had less #COVID19 cases to date.

Thread . Lots of positive energy towards #ContactTracing from the tech sector, but not sure we need more/better apps for #COVID19 #ContactTracing is much more than just maps, which we also have plenty of. Hybrid tech:human approaches are essential. #ClickandMortar
Identifying and mapping #contacts is a core goal of #ContactTracing but there are important details that cannot be scraped from passive location & interaction data such as the specific nature of the contact individuals, disease and clues about #viral load & infectious dose
These details cannot be adequately determined without collecting data that would clearly be an invasion of #privacy unless adequate #InformedConsent is utilized with EVERY subject and #contact.

#Thread on symptomatic, pre-symptomatic, asymptomatic #transmission. A symptomatic #COVID19 case is a patient who developed signs & symptoms compatible with #SARSCoV2 #coronavirus #infection. Symptomatic transmission refers to transmission from a person experiencing symptoms.
Data from published epidemiology & virologic studies provide evidence that #COVID19 is PRIMARILY transmitted from symptomatic people to others in close contact through respiratory droplets, by direct contact with infected persons or by contact with contaminated objects & surfaces
This is supported by experiences shared by WHO technical partners & presentations by Ministries of Health. Data from studies that have collected repeated samples from patients show that #COVID19 shedding is highest in upper respiratory tract early in the course of the disease.

I study the illness #ME/CFS - a neuroinflammatory condition that usually begins w/ a #viral infection (sometimes a #respiratory infection!). I have argued for years that infecting #pathogens tied to the illness might persist in the central nervous system (CNS) of such patients
2/ While I realize this article is talking about acute #infection and #COVID19 - it explains that #SARS-CoV-2 “might infect neurons, raising questions about whether there could be effects on the brain that play a role in patients’ deaths:”

3/ Matt Anderson, (one of my favorite Beth Israel pathologists!) states, “Some of the purely respiratory symptoms that you might attribute to the #disease, the inability to get air into the #lungs, might actually be defects in #respiration controlled by the #nervous system.”

I have a grievance regarding my mom's care during her bout w/#sepsis. #dementia

At the end of October 2019, my husband & I awoke to find mom's breathing labored, & her mumbling to let her sleep. We let her rest. A fever spiked within the hour, & suddenly she became deathly ill.
She was twitching, sweating profusely, in full rigors, & unable to sit up/speak well. She looked TERRIFIED. I thought she had a stroke. Turns out, it was a high fever from an infected gallbladder. She landed in the #ICU. ALL her vitals were jumping around. It was horrible.
She landed in the ER & eventually the ICU. Turns out she had #sepsis from the infected gall bladder. The ICU's 1st step? Changing her meds around INSTANTLY. Specifically they rescinded ALL #opioids abruptly. After over a decade on them, she was thrown into acute withdrawal in ICU

#Campaign in tweets - @Guardicore Labs in a new tradition; we find the attacks, you get to know them and learn the attackers' tricks and techniques. This time, let's get familiarized with "Lemon_Duck", a #cryptomining campaign involving a sophisticated #propagation tool. 🍋🦆
Before we start: all scripts, binaries and IOCs are available on our github repository. In addition, malicious IPs, attack servers and domains appear on @Guadicore Cyber Threat Intelligence portal. You're welcome to take a look :)

Lemon_Duck starts by breaching machines over the #MSSQL service or the #SMB protocol. We'll focus on the MS-SQL flow. Once inside the machine, the attacker enables #xp_cmdshell to run shell commands. It will take only a single command line to trigger the rest of the attack.
